| Objective: Analysis and comparison of the therapeutic effect of the treatment of pilon fracture in minimally invasive locking plate fixation and traditional open reduction anatomical plate fixation , this paper discusses the clinical application of minimally invasive locking plate fixation for treatment of pilon fracture.Methods: Retrospective analysis 32 cases of patients with pilon fracture who were treated with Minimally invasive locking plate fixation or open reduction anatomical plate fixation as the research object in our hospital (the 2nd affiliated of dalian medical university) between March 2008 and June 2010. Group A was Minimally invasive locking plate fixation,there are 15 cases in Group A,11 cases of them are male, 4 cases of them are female; Group B was open reduction anatomical plate fixation,there are 17 cases in Group B,12 cases of them are male, 5 cases of them are female.All the patients are at an average of 38.6-year-old(ranged 21-63). Among them, 12 cases for falling injury, 18 cases for traffic accident injury and 2 cases for sprain. According to allgower classification standard Ruedi-Allgower on points type:9 cases for type I, 15 cases for type II, 8 cases for type III.Recorded the time of the operations, early postoperative incidence of complications, fracture healing rate, ankle function in both groups, to compare by statistical analysis.Results: 32 patients were followed up,1 lost in Group A,2 lost in Group B, 29 patients have completly followed up datas, All patients were followed up for 8-24 months,average 17.2 months. The operation time of Group A was 65.71 minutes on average, The operation time of Group B was 68.33 minutes on average,there was no significant difference (p﹥0.05). According to radiology Burwell-harnley to fracture reduction of evaluation, the situation between two groups was no significant difference (p﹥0.05). 7.10% of Group A had early postoperative complications, Group B was 20.00%.Early postoperative complications have difference between two groups of patients (p﹤0.05). According to Olreud-Molander score to compare the function of ankle,the score of Group A was 91.78, Group B was 83.67. The function of ankle have difference between two groups of patients (p﹤0.05).Conclusion: there was no significant effect on operation time and surgical fracture restoration situation by Minimally invasive locking plate fixation. Minimally invasive locking plate fixation have less early postoperative complications,shorter fracture healing time, better ankle function restore. It is one of the ideal choice for treatment of pilon fracture. |
