Objective:1To investigate the distribution and antimicrobial resistance of Enterobactercloacae Taiyuan part of the hospital èto supply referrence for clinical use of drugsrationally.(2)To investigate the prevalence of AmpC and CTX-M producing Enterobactercloacae isolates in the region for control nosocomial infection provide the scientific basis.Methods49strains of Enterobacter cloacae were collected from Shanxi Medical UniversityFirst Hospital and Shanxi Province People's Hospital, They were all isolated from thepatients'specimens from Auguest2009to June2010.antimicrobial susceptibility were tested byK-B method.The data were analyzed by WHONET5.4software.ESBLs producing isolates wereconfirmed by phenotypic confirmatory tests as recommended by the CLSI, PCR sequencingwere used to determine the genotype of the CTX-M AmpC enzymes. And gene sequencinganalysis.Results:A total of49clinical isolates of Enterobacter cloacae were collected, and were mainlydistributed in departments of Neurosurgery32.3%è Respiration32.6%,Orthopaedics14.3%è51%from soutum tract;The Enterobacter cloacae isolates showed the lowestresistance rate (8.2)to meropenem,the resistance rates to antimicrobial agents from to low to highwere cefepime (46.9%) Amikacin53.1%Trimethoprim/Sulfa55.1è%the resistancerates to other antimicrobial agents were high than60%;49isolates59.2%were ESBLsproducing.31of49producing Enterobacter cloacae were confimed to produce AmpC genes byPCR,By analyzing the sequencing with Acinetobacter baumannii6013113GenBank No.:NZGL891561in GenBank, coincidence rate is95%. Twenty of49Enterobacter cloacae wereconfimed to produce CTX-M genes by PCR. By analyzing the sequencing with Escherichia coliplasmid pEK204GenBank No.: NC013120in GenBank, coincidence rate is99%.Conclusions: Enterobacter cloacae is the major pathogenic bacteria in nosocomial infection, themultidrug resistance is very serious and its resistance mechanisms are very complicated.(1)Enterobacter cloacae were recovered from patients in the respiratory, the isolates were mainlyrecovered from sputum tract. Enterobacter cloacae resistance rate remained at a relatively highlevel. ESBLs and AmpC producing Enterobacter cloacae are prevalent.(2) CTX-M producingEnterobacter cloaceae is higher. Should strengthen CTX-M enzyme monitoring, to prevent thespread of the plasmid resistance.
