Objective To discusses the practical application of equilibrium pressure penumatic tourniquet pressure, and study the effect of equilibrium pressure penumatic tourniquet on haemodynamics and metabolic changes and reperfusion injury.Observe the hemostatic effect and penumatic tourniquets vice damage.Methods In accordance with the random number table,40 cases of healthy adult volunteers were randomly divided into two groups:equilibrium pressure pneumatic tourniquet group (A1)and conventional tourniquet group (A2), with a Doppler detector, brachial artery tourniquet pressure of pulse disappeared were measured in all groups; Using statistical software SPSS13.0,two independent sample test line volunteers meant test, to verify all the advantages of equilibrium pressure pneumatic tourniquet. In accordance with the random number table,80 cases of surgery about knee and ankle were randomly divided into two groups, both equilibrium pressure pneumatic tourniquet group (B1) and conventional tourniquet group (B2) under the operation. (B1)group of hemostatic pressure were set to normal test-takers+25 kPa. (B2) group of hemostatic pressure were set to normal test-takers+25 kPa.Creatine kinase (CK), lactate dehy drog enase (LDH) and femoral vein blood gas were measured before inflation and 1,3,5,15min after deflation of a pneumatict high tourniquet. Hemostasis effect was evaluated by surgeons.The surgeon observed hemostatic effect, duration and complications.Results Volunteers test results, brachial artery occlusion pressure, A1Group compare with A2 Group:t=18.6177, P=0.000, p<0.01. A1 group pressure is less than A2 group. There were no statistics difference between two groups on BP, HR, CK, LDH and blood gas (P> 0.05). Clinical trials:B1 group of Mean Rank=37.40, B2 Group Mean Rank= 43.60, p=0.019,P<0.05, B, group is better than B2 group of haemostasis. There were four case occurred tourniquet complications in B2 group. There were no case in B, group.There was significant difference in the injury rate between 2 groups (P<0.05).Conclusion The equilibrium pressure pneumatic tourniquet can reduce oclusion pressure in upper limb operation, providing good bloodless field, avoiding tourniquet complication.
