| Objectiv The purpose of this study was to describe nutritional status, qualityof life and immune function of patients with locally advanced nasopharyngealcarcinoma who are undergoing concurrent radiotherapy and chemotherapy andto comparative and explore the correlation among the three in different timeperiods.Method Patients diagnosed with patients with locally advancednasopharyngeal carcinoma who were admitted for planned chemotherapy wereapproached using a convenient sampling method, six instruments wereemployed for data collection. Demographic information sheet was used tocollect the subjects' demographic characteristics.The EORTC QLQ-C30and EORTC QLQ-H&N35was used to assess quality oflif, the MininutritionalAssessment, Biochemical indicators was used to measur nutritional status,andDetermination of immune function.136hospitalized patients were asked to complete questionnaires Twelve time points: at1day,7days,14days,21days,28days,35days,42days,49days,56days, atfer treatment3months and6months of concurrent chemotherapy and radiotherapy. Descriptiveanalysis and were performed using the Statistical Package forSoeialSCience(SPSS)13.0, Data was collected from October2010to March2011atGuangxi Medical University Cancer Hospital.Results1, There are significant differences between nutritional status at different stagesof treatment for Patients with locally advanced nasopharyngeal carcinoma(P<0.05). Treatment atfer3months and6months the patient's nutritionalstatus difference was statistically significant (P <0.05).2, There are significant differences between quality oflif at different stages oftreatment for Patients with locally advanced nasopharyngeal carcinoma(P<0.05). End of treatment atfer3and6months the patient's quality of lifeduring treatment compared with statistical significance (P <0.01).3, There are significant differences between immune function at different stagesof treatment for Patients with locally advanced nasopharyngeal carcinoma(P<0.05). Atfer treatment, the CD4/CD8ratio in the three patients withcellular immunity and humoral immune IgM and during treatment differencewas statistically significant (P <0.05) in cellular immune CDS, CD4, CDS, andhumoral immune IgA, IgG, fivewith no signiifcant differences in the treatmentperiod (P>0.05).4, Univariate analysis: Locally advanced nasopharyngeal carcinoma concurrentradiochemotherapy the quality of life of chemotherapy patients in different timeperiods and negative correlation with the nutritional status: fatigue (r=-0.672), nausea and vomiting (r=-0.388), pain (r=-0.454), insomnia (r=-0.386), lossof appetite(r=-0.596), constipation (r=-0.286), diarrhea (r=-0.218), economicdififculties (r=-0.555), dry mouth (r=-0.430), saliva viscosity (r=-0.177).painmedications (r=-0.166), nutritional support (r=-0.213); Locally advancednasopharyngeal carcinoma over the same period to put the time of thechemotherapy patients quality of life and nutritional status was positivelycorrelated: Physical function (r=0.619), role functioning (r=0.710), emotionalfunction (r=0.541), cognitive function (r=0.701), general health status (r=0.727), swallowing (r=0.383), feeling(r=0.375), social eating (r=0.310),social contact (r=0.372), weight change (r=0.107). Locally advancednasopharyngeal carcinoma over the same period to put the quality of life andnutritional status of chemotherapy in patients with different time periods are notrelevant: Trismus (r=0.030), cough (r=-0.061), feeling sick (r=-0.045).Locally advanced nasopharyngeal carcinoma concurrent radiochemotherapy IgAin different time periods of chemotherapy in patients with immune function (r=0.354) and IgG (r=(0.599), IgM (r=0.341), CDS (r=0.377) and CD4(r=0.240)and CDS (r=0.367) and positively related to nutritional status; CD4/CD8(r=-0.159) is negatively correlated with nutritional status.5, Die multivariate Analyse: Overall health status, cognitive function, painmedications, emotional function and IgG, fatigue, diarrhea, IgA, and dry mouth,saliva viscosity, loss of appetite, insomnia involved in building a multipleregression model, a linear relationship with the nutritional status, Affect thenutritional status of significant factors.Conclusions1.The nutritional status of patients with locally advanced nasopharyngeal cancer patients throughout the treatment period showed a gradually declining trend oflong duration,3months and6months atfer treatment there is still potentialmalnutrition and malnutrition cases exist.2.Locally advanced nasopharyngeal carcinoma patients to put the same periodthe overall quality of life during chemotherapy and immune function continuedto decline;6months atfer the end of treatment, quality of life in patients with animproving trend, but failed to recover to pre-treatment; immune functionimprovement is not obvious.3.Locally advanced nasopharyngeal carcinoma over the same period to put thenutritional status of chemotherapy in patients with general health status,cognitive function, pain medications, emotional function, fatigue, diarrhea, drymouth, saliva viscosity, loss of appetite, insomnia, IgQ IgA, and otherfactorsrelated. |
