AimIn this study, with investigation of patients who has taken gastroscopic examination, we analyzed the types and distribution of upper gastrointestinal diseases, explore the possible factors which may affect the initiation and conversion of upper gastrointestinal diseases, in order to improve preventative and therapeutic methods of upper gastrointestinal diseases with scientific evidence.MethodPatients with a variety of conscious discomfort get to the First Affiliated Hospital of Kunming Medical University in November2010-June2011and exclude the diagnosis is not clear and repeat endoscopy, we were randomly selected2150cases (male1090, female1060, mean age47.60±14.77years) in digestive endoscopy room. All the subjects are used OLYMPUS-XQ240type or OLYMPUS-XQ260electronic gastroscope, we reference to2004version of the "digestive endoscopy" about diagnostic standards and methods of operation to complete routine gastroscopy gastroscopy. The doctor endoscopic biopsy if illness need, Medical Examiner detailed recorded the results of endoscopy, when client review several we selected the greatest result or illness reported as a endoscopic biopsy. Blowing method or enzyme linked immunosorbent assay using HP carrier testing situation. Make use of prepared before the examination unified questionnaire to collect information on subjects including the basic characteristics of the subjects, eating habits of life, social background and economic status. Psychological factors were evaluated using "Hamilton Anxiety Scale". Deal with data used SPSS18.0statistical software and P<0.05standard is regarded as a statistically significant. Observation of upper gastrointestinal endoscopy detected the type of disease and the detection rate of sequencing and look for upper gastrointestinal disease that may affect the development.severity and prognosis of outcome of the main factors.Result①All the2150subjects have the abnormal gastroscopic founding (positive rate100%). There are about40cases of upper digestive tract diseases, in which check out rate>5%of disease species including:1402cases of chronic superficial gastritis (65.21%),551cases of duodenal infiammatory(25.63%),480cases of simple chronic superficial gastritis (gastric sinus)(22.33%),426cases of chronic superficial table sexual gastritis with erosion(19.81%),325cases of duodenal ulcers(15.12%),165cases of reflux esophagitis (7.67%),119cases of chronic superficial gastritis with bile reflux(5.53%), and110cases of gastric ulcer in every period(5.12%).The most frequency of the abnormal founding in gastroscope checking is inflammation(70.84%), the second is ulcer(22.09%), and tumor is at least(7%).②isease group finding:480cases of tiny abnormalities(22.33%),1518cases prominent abnormalities(70.6%), and152cases of malignant diseases(7.07%).③There are15factors which have no statistically significant differences with upper gastrointestinal diseases, they are respectively the nation, blood, marriage, origin, place of residence, occupation, the level of education, cold drinking, hot food, dairy products, taste with spicy, pure, sweet, sour, and working or movement after meal.④here are15factors which have statistically significant differences with upper gastrointestinal diseases, they are respectively gender, age, BMI, monthly income, salted, greasy fried, spicy taste, salt taste, timely diet, drinking tea, smoking and passive smoking, alcohol, HP infection and psychological anxiety.⑤ompare significantly diseases with minor lesion11vulnerable factors are arranged in odds ratio (OR value):HP positive (OR=3.205), elder(OR=2.851), definite anxiety(OR=2.376), smoking(OR=2.123), possible anxiety(OR=2.058), drinking (OR=1.837), male(OR=1.655), middle-aged(OR=1.651), passive smoking(OR=1.474), pickle(OR=1.384). The protective factors are the monthly income of more than3000yuan RMB(OR=0.656), timely diet(OR=0.401), drinking green tea(OR=0.208).⑥ompare significantly diseases with malignant diseases,12vulnerable factors are arranged in odds ratio (OR value):elder(OR=16.423), HP positive(OR=6.731), middle-aged(OR=5.948), definite anxiety(OR=4.584), possible anxiety(OR=4.558), smoking(OR=4.141), drinking (OR=3.421), male(OR=3.147), salt taste(OR=2.519), BMI20(OR=2.408), non-green tea(OR=2.395), greasy fried(OR=2.138), and the protective factors have timely diet (OR=0.335), the monthly income of more than3000yuan RMB (OR=0.243), drinking green tea(OR=0.133).⑦ale, aging, drinking and smoking, HP infection, anxiety and so on have the positive relationship with the variety and severity of the upper digestive tract diseases (P<0.05); women, high income, timely diet, drinking green tea have the negative correlation(P<0.05).Conclusion①Gastroscopic manifestations of patients who received routine gastroscopic examination with abdominal pain and abdominal distension as the main symptoms in the First Affiliated Hospital of Kunming Medical University is abnormal; The most common upper gastrointestinal disease manifestations is inflammation, followed by ulcers and tumors, which belongs to tiny abnormalities, prominent abnormalities and malignant abnormalities.②Physical characteristics, social background, psychological state, eating habits, HP infection and many other factors combined with the development of upper gastrointestinal disease and its severity in patients.③ationality, blood type, marriage, native place, permanent residence, occupation, education, like cold drink/hot food/dairy products, partial spicy, light, sweet, acerbity taste, work after the meal or sport seem to be unrelated the development of upper gastrointestinal tract disease.④Male, increased age, like salted and fried food, drinking and smoking, HP infection, mental disorder are possible risk factors of upper gastrointestinal tract disease; women, high income, eat regularly and drink green tea frequently are seem to have protection to upper gastrointestinal tract disease.⑤We found out that improvement of living standards, maintaining psychological health, abstaining from smoking and alcohol, eating less high salt, fried or salty food, regular diet, healthy diet and habit like drinking green tea, etc. could have great significance on preventing and alleviating upper gastrointestinal disease.
