Clinical Value Of Ultrasound Real-time Tissue Elastography Diffuse Quantitative Analysis Of Technology Assessment Of Hepatic Fibrosis Research

Objective:To evaluate the diagnostic value of real—time tissue elastography with tissue dispersion quantitative analysis software for the assessment of liver fibrosis stage.Methods:91patients with liver fibrosis confirmed by biopsy were included in the study. In all patients(47men and44women).61cases with chronic hepatitis B and30cases with hepatitis C, from18to74years old with mean age of40.0±6.7years, real-time elastography with dispersion quantitative analysis software was performed. At first, the size, shape, involucrum, essence and inside pipe structures of the liver were observed, then stable elastic drawings were gained by elastography probe on the same area performed with liver biopsy and12elastic quantitative parameters, e.g. MEAN. SD.%AREA. COMP. KURT, SKEW. CONT. ENT. IDM. ASM. CORR. LF were recorded automatically, the12parameters were compared with pathological stages of liver biopsies(SO—S4). The differences of12parameters among fiver groups were assessed with rank sum tests for several independent samples; and The Spearman correlation statistics between12parameters and liver fibrosis stages was performed; Then the receiver-operating characteristic curve (ROC curve) was used to judge the diagnositic accuracy and cutoff value of the most correlative parameters for staging the liver fibrosis.Results:The characteristic parameters had statistically significant differences except CORR among fiver groups (P<0.05). The correlation between the other parameters and pathological stages was well, except the KURT (P<0.05). Among this parameters, MEAN, LF and%AREA had the highest related coefficient(r=-0.677.0.662.0.637).The ROC curve was made by MEAN, LF and%AREA to estimate the diagnostic accuracy about distinguishing the fibrotic stage, when S>S0. S>S1. S>S2. S>S3, the areas under the ROC curve were0.832.0.838.0.869.0.856(MEAN);0.830.0.838.0.861.0.831(%AREA);0.813.0.827.0.849.0.820.(LF).And when S>S0. S>S1. S>S2, the cut off value were104.05.88.30.85.15(MEAN);31.72.39.02.41.74(%AREA);3.00.3.26.3.91(LF)Conclusion:It's helpful to evaluate the stage of hepatic fibrosis by the real—time tissue elastography with tissue dispersion quantitative analysis software. The MEAN, LF and %AREA are useful for distinguishing liver fibrosis, depends on their highest correlation with pathological stages of hepatic fibrosis.
