Short-term Investigation Of Operation Therapy For Degenerative Scoliosis

Objective：Analysis of Short-term investigation of operation therapy for degenerativescoliosis.Methods：From August2008to December2011，21patients in our hospital sufferedwith degenerative scoliosis were treated by decompression and correctioninstrumented with pedicle screws through posterior procedure，and fusions wereundergone at the same time．Of them，7cases were fused with intervertebral cages，and the other14cases fused by post—lateral bone grafting. The follow-up periodobservation of symptoms, and reduce the degree of scoliosis correction result, Bonegrafting fusion and complications.Results：Operation time4-6h, an average of5.3h；Mean intraoperative blood losswas1400mL. During the15months follow-up, all the patients showed remarkablerelief from pain and claudication and improved life quality． JOA score bypreoperative6~16points, an average of11.2points, restore to postoperative17to28points, an average of21.07points (P <0.001); Clinical symptoms GaiShanLv51.3%on average The procedures resulted in significantly lowered ODI from (58.3±7.22)%before the operation to (35.1±5.53)%after the operation (P<0.01). A significantimprovement of the coronal Cobb’s angle was achieved after theoperation(21.34°preoperatively vs8.98°postoperatively,(P<0.01), and the lordosisangle was improved from16.14°to26.64°after the operation (P<0.01). All the patientswere followed up, and no implant loosening, displacement or fragmentation, orpseudarthrosis was found at the final follow-up. Linear correlation and regressionanalysis showed that there was a positive correlation between the increase of JOAscore and the increase of the lumbar lordosis angle. Conclusion： For conservative treatment is invalid, and clearly backleg painsymptoms or nerve root compression symptoms and progressive increase or obviousthe instability of the section of the degenerative scoliosis, surgery can get satisfactoryclinical curative effect and deformity correction result, correct choice of the case andoperation method, restore coronal and sagittal balance, reconstruction lumbar lordosisphysiological surgery is the key to success.
