Objective To investigate the dynamic changes and clinical significance of thepre-and post-operation plasma H-FABP in patients with AMI.Methods This study adopts ELISA to assay H-FABP, cTnI, CK-MBconcentrations in plasma at0~6h,9h,12h,24h, and48h after presentation in50patients with AMI Meanwhile, the plasma level of H-FABP in25controls also weredetected at pre-operation coronary artery angiography. The AMI group was dividedinto three subgroups: the death subgroup, the complication subgroup and the non-complication subgroup and they were relatively analyzed.Results1. H-FABP concentrations in plasma of patients with AMI was higherthan that of the controls, respectively31.13±6.72μg/Lvs7.19±2.43μg/L (P<0.01)。and the difference between them has statistical significance(P<0.01).2. The plasma concentration of H-FABP was positively correlated with Gensiniscore, the more obvious the concentration of H-FABP was increased, the higherGensini score.3. Among the three subgroups, H-FABP levels in plasma came to peak in thenon-complication subgroup at4-6hours after presentation, then it returned to normalrange at12-24h after presentation. H-FABP levels in both the complication subgroupand the death subgroup were obviously higher than that in the non-complicationsubgroup at each time point. Their peaks were delayed, remained high and had anincrease trend. Among three subgroups, the different time points, and the interactionof among groups and the different time points, the three differences werestatistically significant(P<0.05).Conclusion1.H-FABP is a specific biochemical marker in the early diagnosisacute myocardial infarction and it is fairly important for the diagnosis of AMI.2.The extension and even continuous rise of its peak are probably related to thedamage of myocardial cell of which many complication cause After AMI. The dynamic change of H-FABP concentration has predictive values in the riskstratification of AMI patients, Which would be used as a predictive marker forassessing the clinical conditions and the development of cardiovascular events, andcan obviously promote coronary artery local lesion occurrence and development. It isvery important clinical significance for guiding clinical early positive intervention andtimely preventing complications to take place.
