Objective To compare the effect of sedation of combined spinal-epidural anesthesia and epidural anesthesia. Methods A randomized controlled trail was conducted on60patients undergoing selective gynecological abdominal surgery.Women were randomly divided into combined spinal-epidural anesthesia sedation group (group SE) and epidural anesthesia sedation group (group E). The sensory block level was controlled between T6and T8,the TCI of propofol was started at a target plasma concentration of1.2mg/L-1,followed by increments0.2mg/L-1at30s intervals until the BIS value was70. The intraoperative BIS value was maintained between65and75with propofol TCI.Observation indexes The propofol effect-site concentration, the modify OAA/S score, the onset dose and total dose of propofol, total dose of lidocaine, the duration of surgery and sedation, the rate of ephedrine and atropine, the volume of intraoperative transfusion and blood loss. Results Compared with group B,the propofol effect-site concentration, the onset dose of propofol, the total dose of propofol and lidocaine were decreased in group A (P<0.05); the rate of ephedrine was increased in group A (P<0.05).There were no diference in the modify OAA/S score, the duration of surgery and sedation, the rate of atropine, the volume of intraoperative transfusion and blood loss between the two groups (P>0.05)Conclusion Combined spinal-epidural anesthesia has an obvious sedation effect compared with epidural anesthesia from the decreased effect-site concentration and dose of propofol.
