Objective:TO investigate plasma Visfatin levels in Type2Diabetic(T2DM) patients with nonalcoholic fatty liver disease(NAFLD) and the relationship between Visfatin and insulin resistance, liver function(ALT、GGT), and also to supply the foundation of the theory to select and assess the risk factors of NAFLD procession.Methods:The fasting plasma Visfatin levels were measured in65T2DM patients(1999WHO diagnostic standards), including35diabetic patients without NAFLD (18males and17females,51.97±12.59years old, DM duration1.18±1.49years),30diabetic patients with NAFLD (16Males and14Females,52.07±12.08years old, DM duration1.83+1.81years) and25healthy individuals(control group). Fasting blood glucose, plasma lipid, insulin, HbAlc and liver function parameters were also measured. Meanwhile we measured the height, weight, waist circumference (WC) and blood pressure. Homeostasis model assess insulin resistance index (HOMA-IRJ) and function of insular B cell (HOMA-BI). Fixed special doctor examined the livers by color Doppler.Results:①The plasma Visfatin levels, HOMA-IRI, WC and TG in diabetic patients without NAFLD were significantly higher than that in control group, but lower than that in diabetic patients with NAFLD(P<0.05).②Levels of DM duration, SBP、DBP、FBG、 HOMA-BI、HbAlC、TC、HDL-C and LDL-C had no difference between two DM groups (P>0.05).③DM duration、SBP、DBP、BMI、WC、FBG、FINS、HOMA-IRI、 HBAlC、ALT、AST、GGT、TC、TG、LDL-C had significant positive correlation with Visfatin(P<0.05), and HOMA-BI、HDL-C had negative correlation with Visfatin (P<0.05).④Multiple regression analysis showed FBG、WC、ALT、GGT were the risk factors of Visfatin.Conclusion:①Visfatin level had negative correlation with HOMA-BI, and had positive correlation with HOMA-IRI.②The plasma visfatin levels in diabetic patients without NAFLD were significantly higher than that in control group.③Visfatin levels in diabetic patients with NAFLD were lower than that in diabetic patients without NAFLD, the reason may be the aggravation of IR and the inflammation of fatty liver tissue.④FBG、WC、ALT、GGT are the risk factors of Visfatin.⑤Visfatin probably has correlation with T2DM, Abdominal Obesity and the progress of NAFLD by insulin resistance mechanism.
