| Objective: Understanding of clinical nurses’ discrimination towards PLWHAs, in order toforming a comprehensive training model for training clinical nurses to reduce nurses’ discriminationagainst PLWHAs and evaluate its long-term effect, So as to enhance HIV prevention effectively andimprove the living environment of PLWHAs.Methods:1. Combined pre-survey results, History information with the Delphi method to built"progressive" training model. The methods including: HIV status notification-Knowledge missionary-regulate nursing behavior-Psychological intervention and contact with patients,2.120clinical nurses were selected from Kaifeng Huaihe Hospital by using stratified. randomizedmethod, They are in different clinical departments from First Affiliated Hospital of Henan University andHuaihe hospital. Then doing pre-research by using self made questionnaire.3. Training those selected nurses by using "progressive" training mode.4. Then compare pre-training survey results with2months,6months after training survey resultsin order to evaluate the effectiveness of the model.5. Statistical methods: analytical methods including descriptive analysis, paired t-test design,variance analysis, Pearson correlation analysis and frequency analysis.Results:1. Forming "progressive" training model: HIV status notification-Knowledge missionary-regulate nursing behavior-Psychological intervention an-contact with patients.2.125questionnaires were distributed in the first survey, Three nurses did not complete theirquestionnaires due to work or family reasons, two questionnaires were excluded because of missing morethan20%of important information in the post-entry verification process,120valid questionnaires werereturned, the effective rate is96%;240questionnaires were used for two and six months after trainingresearch,240valid questionnaires were returned, the effective rates are100%.3. Three experts guided the questionnaire, validity of0.83, internal consistency Cronbach’s a of0.85. 4.120clinical nurses’ mean score: three indicators before training scoring: AIDS-relatedknowledge (50.03±3.94), discrimination attitude (46.83±3.75), nursing-related behavior (35.36±2.96);2months after training score: AIDS-related knowledge (60.06±4.99), discrimination (57.21±5.74),nursing-related behavior (37.10±2.83);6months after training score: AIDS-related knowledge (58.58±4.04), discrimination (51.90±5.20), nursing-related behavior (37.33±2.82).5. Before and two months after, six months after training, three indicators score were statisticallysignificant (P <0.05).6. Pairwise comparison of before and two months,six months after training, three index scoresare significantly different (P <0.05); in terms of nursing care behavior, comparing two months with sixmonths after training, there is no significant statistic meaning. P=0.90, knowledge score results shows P=0.04; attitude score, P=0.00.7. Qualitative research showed: three factors lead to HIV discrimination: the first is ‘Helpless’felling for current nurse occupational status. Second ‘fear’ which closely related to occupational exposure.fearing of being infected; Third: stigma label which related to "herd following mentality ",‘commercialsexual behavior’ and ‘drug injection’.8. Combining the first-order frequency synthesis and total frequency analysis showed: the currenttop three ways of sources of AIDS prevention and control knowledge areâ‘ TV (51.67%),â‘¡the network(12.50%),â‘¢brochure (9.17%); nurses want the top three ways of sources areâ‘ hospital organization oftraining activities (57.50%),â‘¡expert lectures (12.50%),â‘¢promotional materials (7.50%).Conclusions:1. Compared with traditional training methods, the progressive integrated training model’slong-term effects are good, the cost is low, moreover, this model is effectiveã€easy to operate which caneffectively reduce clinical nurses’ discrimination towards PLWHAs, This model can bring nurses into fullplay in the AIDS prevention tusk and to promote social forces to deal with the spread of AIDS epidemic.2. Strengthen HIV-related knowledge and missionary will help to consolidate the training effect. |
