Objective:To evaluate the role of echocardiography in prognosis of complete transposition of great arteries with intact ventricular septal (TGA/IVS) after arterial switch operation (ASO).Method:Forty-nine patients with TGA in our hospital between January2007and December2011were enrolled in this study.All the patients underwent echocardiography and ASO. Echocardiography visualized the cardiac anatomy.The indexes of heart function, including left ventricular internal dimension in diastole (LVIDd), left ventricular posterior wall dimensions(LVPWd), intervernicular septum in diastole(IVSd),left ventricular ejection fractions(LVEF) and left ventricular fractional shortening(LVFS), was measured by echocardiography.Then calculated the left ventricular mass index. The surgery records and postoperative outcomes were also collected. The echocardiography diagnoses were compared with the surgical findings. Results:Four patients died during or soon after procedure, and the other45were discharged, the survival rate was91.84%. According to the surgery records, diagnostic accuracy of TGA/IVS and TGA with other anatomical malformation by echocardiography was100%,91.84%respectively. For children with TGA/IVS,left ventricular ejection fraction and left ventricular shorting fraction decreased gradually with age(P<0.05),the death group’s LVEF and LVFS were lower than the survival group’s. Low LVEF (P<0.05), low LVFS(P<0.05), low left ventricular mass index(P<0.01),interventricular septum partial to the left(P<0.05), accompanied coronary abnormalities(P<0.05) were the risks of ASO.Children with small diameter of the patent ductus arteriousus(PDA)(P<0.05),low LVFS(P<0.05), low left ventricular mass index, accompanied coronary abnormalitied(P<0.01),needed longer time in hospital stay.Conclusion:Echocardiography can accurately diagnosis complete transposition of great arteries accompanying other anatomical malformation and measure the patients’ left ventricular function, Preoperative left ventricular function was relevant to the prognosis of patients who would undergo arterial switch operation.
