| Objective: To investigate the effect of intracoronary injection ofnitroprusside on no-reflow phenomenon(NRP) of primary percutaneouscoronary intervention(PCI).Methods: Sixty-eight acute myocardium infarction patients with STsegment elevation(STEMI) who had no-reflow phenomenon after primary PCIwere included in the study.All the patients were administered intracoronarybolus nitroglycerm(200μg).Coronary arteriography was rechecked5min afterinjected.Sixty-one patients who had no better forward flow of Thrombolysis InMyocardial Infarction(TIMI) grade were randomized to nitroprussidegroup(intracoronary nitroprusside,n=31) and tirofiban group(intracoronarytirofiban,n=30).Coronary arteriography was rechecked5min afterinjected.Targets to be observed consisted of corrected TIMI framecount(CTFC),TIMI flow at5minute post-medication.safty end-points andincidence of major adverse cardiovascular events(MACE) of10daypost-PCI.Observe differences of brain natriuretic peptide(BNP)7daypost-PCI.Inaddition,Cardiac structure and cardiac ventricle systolic functionincluding left ventricular medial diameter (LVMD),left ventricular ejectionfraction(LVEF) was evaluated after10days.Results:Compared with control group,TIMI3Grade flow and cTFC innitroprusside increased significantly(P<0.05).There was significant differenceof BNP between these two groups(P<0.05).UCG demostrated that there weresignificant differences in LVMD,LVEF of10days.Conclusion:The administraction of intracoronary nitroprusside is effectivefor STEMI patients with no-reflow after PCI. |
