Comparison Of Simple Pemberton’s Ilium Osteotomy And Combination With Unter-trochanter Derotation-varisation Osteotomy In Treatment Of Developmental Dysplasia Of The Hip

Objective: To retrospectively compare the therapeutie effects of SimplePemberton’s ilium osteotomy and combination with unter-trochanterderotation-varisation osteotomy for developmental dysplasia of the hip.Methods: We have collected the141DDH (developmental dysplasia ofhip)patients data from janury,2004to janury,2009, which is included bysimply Pemberton or together with under-trochanter derotation-varisationosteotomy. Among them57patients were undergoed simply Pemberton’s iliumosteotomy(simply Pemberton group) including male12cases, female45casesand age ranging from3-8years old.84patients were treated by Pemberton’silium osteotomy in combination with under-trochanter derotation-Varisationosteotomy(combinations group) including male15cases, female69cases andage ranging from4-9years old. Assess the complications of posteroperationand The clinical results and x-ray measurements of all the patients wereanalysed by Severin and Mckay separately2years after the oparations.Results: The simply Pemberton group was followed30-52months(mean38M) and the combinations group was followed32-51months(mean37M).However, we found that there were7patients of dislocation,4patients ofavescular necrosis of femoral head and5joint stiffness in the simplyPemberton group. on the contract, there are no dislocation,contraction of thelimb and ANFH but2joint stiffness in the combination group.After2years ofposteroperation, the total effective rate of the clinical results and x-raymeasurements analysed by Severin and Mckay of combination group are apparently higher than the simply Pemberton group(P<0.05). Combinationsgroup is better than the simply Pemberton group. The results of thesimplyPemberton group assessed by McKay shows: better27hips, good18hips,moderate12hips. the total effective rate is78.94％. The results oftheCombinations group assessed by McKay shows:better52hips, good25hips,moderate7hips. the total effective rate is91.66％. The results of thesimplyPemberton group assessed by Severin X-ray shows: better27hips,good15hips,moderate15hips. the total effective rate is73.68％. The results oftheCombinations group assessed by Severin X-ray shows: better53hips,good22hips,moderate9hips.the total effective rate is89.28％.Conclusion: Comparing to the simply Pemberton group,combination groupin treating the DDH of childhood has the more better clinical outcomesespecially in reducing the complications of posteroperation and restoringfounctions. The patients of combination group have the higher satisfaction afteroperation.
