Efficacy comparison between cannulated screw with dynamic hip screw blade in treating femoral neck fracturesObject:To compare the curative effect of dynamic hip screw blade and cannulated screw in treating patients with non-pathological fresh femoral neck fracture and to provide the basis for the choice of surgical procedure.Methods:We reviewed and analyzed58patients with non-pathological fresh femoral neck fracture treated by using closed reduction internal fixation with dynamic hip screw blade and cannulated screw between February2010and August2011in our department. The cannulated screw group (A group) included34patients, the DHS blade group (B group) included24patients. The period of follow-up from6month to24months, and the average period is15.2months. In-hospital data were collected retrospectively and the postoperative clinical situation were analyzed. At the same time we use the SPSS19.0to analysis the correlativity between the two groups. The A group:included14males,20females; aging15-85years, mean54.6years; height157-177cm, mean167.1cm; weight40-87.5kg, mean59.5kg; the subcapital fracture type10, the transcervical fracture type18, the base of the neck type6. The B group: included8males,16females; aging19-86years, mean55.3years; height155-189cm, mean166.1cm; weight48-85kg, mean63.4kg; the subcapital fracture type5, the transcervical fracture type15, the base of the neck type4. There was no significant difference in the baseline characteristics between the two groups (P>0.05). The main outcome measures included operation time, incision size, operation blood loss, hospitalization days, part weight bearing time, complication and clinical efficacy.Results:①Operation time, Incision size, Operation blood loss comparison:The A group,59.4±20.2min,4.1±0.6cm,25.9±20.2ml; The B group,88.6±22.9min,12.1±1.2cm,156.7±107.1ml; There were significant differences between the two groups. The A group had less operation time, incision size, and operation blood loss.②Hospitalization days, Part weight bearing time, Complication, Clinical efficacy (harris score):The A group,5.5±3.7d,87.2±34.7min,11.76%,87.2±9.2; The B group,6.5±3.8d,80.1±38.1min,4.17%,91.9±9.8ml. There was no significant difference between the two groups.Conclusion:①The surgical treatment of femoral neck fracture by using cannulated screw has advantages of less operation time, incision size operation blood loss and smaller trauma.②Both of the two groups achieved satisfactory clinical efficacy.
