ObjectiveThe Syndrome in TCM of Chest Obstruction with Pain(胸痹证)is defined to be a kind of disease with heartache. The heartache is induced directly by heart pulse impassability which can be aroused when human body is attacked by all kinds of harmful factors or dysfunction of Zang and Fu. In basic researches, the Syndrome in TCM of Chest Obstruction with Pain is equal to myocardial ischemia in western medicine, because they are similar in pain position, character, symptom and prognosis.Ju Pi Tang (recorded in<Essential Medical Treasures of the Golden Chamber>) consists of Pericarpium Citri Reticulatae and Rhizoma Zingiberis. The long term of using experience shows that Ju Pi Tang has effects on cardiovascular disease. In our laboratory, we set out researches on its extract which is compounded by Pericarpium Citri Reticulatae Exstract and Rhizoma Zingiberis Extract(1:2,v:v), named Pericarpium Citri Reticulatae and Rhizoma Zingiberis Extracts(PRE). The results of the previous studies show that PRE can definitely protect the ischemia myocardium. However, those studies were just conduct on one kind of myocardial ischemia models and the indexes being detected were inadequate to make a fully and systemic illustration of its effect on myocardial ischemia prevention and treatment. Thus, this study is designed to view the protective effect systematacially in three kinds of myocardial infarction models. The result can be the base for the exploiting of PRE to be a drug for myocardial ischemia.Method1The myocardial protective effect of PRE on acute myocardial infarction (AMI) rats induced by the ligation of left anterior descending coronary artery (LAD)Rats from all groups were medicined by garage for8days and were operated the surgery of legating the LAD (the sham group just underwent the thoracotomy) one hour after the gavage in the seventh day.24hours later, recorded the ECG of the rats to observe the elevation of ST-segment, and then collected the haemodynamic parameters to analysis the ventricular function. Later, took the blood sample from abdominal aorta, isolated the serum for biochemical assay, including CK, LDH, SOD, GSH, GSH-Px and MDA, and make a full view at the capacity of clearing oxidates for the myocardial cell. At last, removed the heart, transected into pieces of lmm and immerse in1%tetrazoline solution to react for5min, then calculate the infarct size.2The role of PRE in isoproterenol (ISO)-induced myocardial ischemia (MI) rats.All rats were given drugs for8days, and injected subcutaneously with ISO to induce myocardial ischemia at one hour after administration at the6th and7th day(except the blank control group). ECG was record24hours after the last injection of ISO and the elevation of ST-segment was considered. Then, collected blood samples from abdominal aorta, isolated the serum for biochemical assay, including CK, LDH, cTnT, SOD, GSH, CAT and MDA, which results can be used to analysis the level of damage in myocardial and the anti-peroxidate ability of the myocardial cell.At last, excised the heart, cut down the distal portion and fixed in10%formalin for subsequent hematoxylin-eosin(HE) staining., and then observe the structure of the ischemia myocardium3Effects of PRE on hemarheology of blood stasis model ratsAll rats were treated by gavage in consecutive8days. At the6th and the7th day, all animals except the normal control group were injected subcutaneously adrenaline (0.8mg/kg) and treated with iced water to induce blood stasis. The blood samples were collected for the detection of hemarheology, platelet aggregation, as well as the contents of endopthelin-1(ET-1) and nitric oxide (NO) in plasma. ResultsOn one hand PRE can strengthen the activation of peroxidase, such as CAT, GSH, GSH-Px and SOD that shorten the accumulation of MDA and the death of cardiocytes, then maitain the structure of the myocardium.. On the other hand, it can improve the function of the left ventricle. What’s more, PRE can also reduce blood viscosity, hematokrit (HTC) and erythrocyte sedimentation rate equation K’s value(ESRK) to improve the hemarheology indexes and inhibit the platelet aggregation when blood stasis occurred.ConclusionPRE can strengthen the body hypoxia tolerance, and improve the compliance for heart. It also has capacity to promote blood circulation and prevent blood clots. So, PRE has effect on prevention, treatment and improve the prognosis for Chest Obstruction with Pain.
