Influence Of Special Treatment Of Meibomian Land On The Conjunctival Sac’s Icroorganisms Before Cataract Surgery

Purpose Prospective study the effect of the conjunctival sac’smicrobial condition after special treatment of meibomian gland includingeye lid fomentation、massage and clean palpebral margin，to explore moreand more safe preparation before ocular operation.Methods87cases of90eyes who needed the cataract surgery dividedinto three groups randomly. A (blank group： without anytreatments), B (control group：routine preparation preoperative includinguse of antibiotic eyedrops+cut lashes+rinse tractus lacrimalis andconjunctival with isotonic Na chloride), C (treatment group：routine preparation preoperative+special treatment of meibomiangland). There were25cases of26eyes,29cases of29eyes,33cases of35eyes respectively. Specimens of the conjunctival sac were collected at theadmission (A group)、at the start of the surgery and at the end of thesurgery(B、C groups)，and sent to bacterial culture.Results1.microbial culture positive rate of the conjunctival sac：in group A was53.85%，at the start of the surgery and at the end of thesurgery, the positive rates were10.34%、27.59%and2.86%、5.71%respectively in the group B and group C. Microbial culture positive rate ofthe conjunctival sac was decreased obviously in the group C(meibomiangland received the special treatment)，there was statistical significance（P<0.05）.2.There wasn’t statistical significance between the microbial culturepositive rate of the conjunctival sac and date of antibiotic eyedrops uesdbefore surgery、time of the treatment of meibomian gland、duration of thesurgery（P>0.05）.3.The kinds of the microbial culture strains：28stocks5generatotally，fungus couldn’t was detected.Conclusions1.There was a certain bacteria in the conjunctival sacof the normal population；routine preparation preoperative could reduce themicrobial culture positive rate of the conjunctival sac， and reduced furtherafter special treatment of the meibomian gland.2.On the condition of reduce the local use of antibiotic eyedrops，added the special treatment of the meibomian gland before cataractsurgery，also could gain the effect of clean operative field and reducebacteria of conjunctival sac，thereby lower postoperative endophthalmitisrate.
