| Objective:To compare clinical efficacy of posterior lumbar interbody fusion with autologous bone or Cage for treatment of lumbar spinal stenosis on the basis of the pedicle screw fixation.Methods:A prospective study was carried out and36cases of two adjacent section of lumbar spinal stenosis were accepted by the same selectioncriteria, they were randomly divided into two groups. Group A (18cases) were done through posterior lumbar interbody fusion with autologous bone on the basis of the pedicle screw fixation system. Group B (18cases) were done through posterior lumbar interbody fusion with Cage on the basis of the pedicle screw fixation system. Compare operation time, volume of bleeding, hospital stay, complication occurrence, as well as following markers wereobserved preoperatively and at the1st month,3rd month,6th month and12th month postoperatively:1. The Japanese Orthopaedic Association(JOA) score.2. take X-ray film to observe disc space heigh.3. the assessment of the integration on the use of standard SUK. All data were analyzed by SPASS13.0.Results:1. two sets of operation time, blood loss, hospital stay and the incidence of complications was no significant difference.2.Significant changement of JOA score was founded in each group pre- and post-operative. But it was no significant difference between two groups;3. Significant changment of disc space height was observed in each group pre-and post-operative. It was also different between two groups that the disc space height lost more significant in group A.4. In each group, angle of interbody movement appeared significant changment comparision pre-and post-operative while comparing two groups, angle of interbody movement and the rate of fusion weren’t significant different.Conclusion:operation time, blood loss, hospital stay and the incidence of complications, angle of interbody movement, The time of fusion, the fusion rate and catabatic grade of clinical results are no significant difference between two groups. Autologous bone as the same as Cage can be used for interbody fusion for treatment of adult instability of lower lumbar and good clinical outcome can be achieved,no inununoreaction And autologous bone is low-cost. But there is significant loss of disc space height in patients utilizing autologous bone comparing with Cage. |
