Gamma Nail Fixation And Hemiarthroplasty In Elderly Intertrochanteric Fracture Of Comparative Analysis

Objective:Through comparative analysis of internal fixation with Gamma nail and artificial thigh bone Replacement in elderly intertrochanteric fracture, explore the reasonable methods of above two operandi.Method:Collect124older patients with intertrochanteric fracture from January2004to February2012, including49males and75females,divided into internal fixation group(66cases,72～86years old, mean age79.1±2.15years old),the follow-up time is3months to30months(mean18months) after operation and artificial thigh bone replacement group(58cases,73～89years old, mean age81.5±3.83years old) the follow-up time is3months to30months(mean19months) after operation.The bone fracture is classified into type Ⅱ (18cases), type Ⅲ(40cases), type Ⅳ(50cases), type Ⅴ(16cases), according to Evans typing, and most of them are unstable. During operation blood loss volume, postoperative complications and admission days,to score two groups in Harris standard was used.At last, compare above items. The data was recepted statistical analysis through spssl7.0.Results:The all124cases took3～30months follow-up time. mean age80.3±2.99years old. Through comparing the operative time, blood loss in operation, admission time, the two groups were no significantly differenct (P>0.05); the postoperation was also no significant different according to harris scoring standard.The postoperative complications of two groups were statistical difference(P<0.05).The time of beginning movement of two groups were statistics difference(P <0.05).Conclusion:Compared with Gamma nail and Femoral head replacement, the effect of femoral head replacement to the intertrochanteric fracture in elderly patients was satisfied, which can make the patients early ambulation, resume the activity ability before the injury as soon as possible, to reduce the bed time or else which can cause complications, then can reduce a patient’s risk of dying and to improve the patient’s quality of life. But it need rigorous operation indication.If the bone conditions are very good and the stability of intertrochanteric fracture Gamma nail was also abetter alternative procedures.
