| Objective:This study combines the observation of clinical cases, compare with the more technique of resetting a small splint fixed and LCP two methods for elderly patients respectively Colles fracture of the recent curative effect, providing more clinical theoretical basis for conservative treatment for elderly patients with fracture Colles to more effectively guide the clinical treatment of the distal radius fracture type straight, and broaden the application scope of the fixed use of small splints.Methods:57cases of elderly patients with distal radius fracture type straight as the research object who were from Hubei Province Chinese Medicine Hospital (garden hill school district) and Wuhan General Hospital of Guangzhou Military Region in March2010to September2011, were conforms to the standard and material completely. Use parallel control random number sub-table points, manipulative reduction and a small splint fixation have30cases in the treatment group, and the locking compression plate screws nails fixed (LCP) have27cases in treatment group. The two groups of patients were taken radiographs of the wrist1:1standard lateral at resetting before and resetting after at three time points of1,4,6weeks after resetting,and they were measured and record of palm dip, ulna deviation, radial relative height, they were guided function exercise, which combined with the X-ray from fracture of the specific and the Bunger such as the Colles fractures after anatomic score, the comprehensive evaluation of anatomic reduction of the two methods.After6weeks, the two groups of patients evaluation short-term curative effect with the Garland-Werlery rating system evaluation wrist joint function. According to the clinical efficacy, the use of SPSS15.0statistical software to analyze and process data(assume that P<0.05was considered statistically significant difference), and concluded that analyzing and processing data.Results:All cases were followed up and the time between2months with four months, the average follow-up period was3months. After six weeks, we concluded that all patients have got bone healing. All patients of small splint in treatment group were removed spl ints within4weeks after resetting fracture. The statistical analysis of the data, compare small splint treatment group with the LCP treatment group, the two groups of dissection score have no statistically significant fracture, the two kinds of methods have a certain of reliability; Two groups of patients were in6weeks after according to Garland-Werlery wrist functional score have no statistical significance, the overall probability of the two groups of treatment effect is the same, similar to the overall wrist function were got to satisfactory effect. In this study, tow patients of experimental group in small splints external fixation for fracture displacement after again hospital after reduction manipulation do small splints external fixation, three patients gimmick reset after the small splints fixed appeared tension sex blisters, and after treatment all got bone healingConclusion:The technique of resetting small splints external fixation treated the elderly patients with Colles fracture is reliable fixation with recent symptoms, and embodies the "combining static" the treatment principles and keep the balance after the plastic force system, is a positive move of elastic fixed mode. From the physiological function of limb fracture, it’s based on the principle of kinematics with the "flesh is linked together tendons and bundle bone" theory. Through tied to the collection of pressure with the splint, and the leverage force of splint on the injured limb, pressure pad side to prevent or shift correction of residual stress, reasonable functional exercise compressive stress, muscle coordination activities intrinsic motivation, the role of appropriate application of timely and reasonable functional exercise. The formation of a relatively stable equilibrated system of forces, so that the body due to internal dynamics due to fracture unbalance restored relative balance in order to maintain the entire complex after the bit and maintain fracture stability of the entire complex.The two groups of patients with distal radial fractures were got bone healing when they referred after6weeks.They got the satisfaction score of anatomy and wrist function score. LCP treatment group after the four weeks after the short term to restore the anatomical reduction,which was superior to the experimental group, but there was no significant difference after6weeks, indicating that the for distal radius fracture type unbend, small plywood external fixation and LCP two kinds of methods are satisfied treatment effect. Considering the small splint fixation with non-invasive, easily obtained, easy to operate, inexpensive, relatively few complications and many other advantages to conform to this stage of medical reform trends, suitable for use in primary health care sector and the general region. It is a kind of effective treatment methods, more worthy of clinical promotion. |
