It brings many difficulties that there are some dysfunctions about insulin resistance and insulin secretion after burning seriously for the treatments of burning nutrition. Nowadays, the existing researches reveal that it is the key point that the signals of receptor insulin defects pathways leading to insulin resistance after burning seriously. SIRS and TNF-α release a large amount of Inflammatory mediators and FFA significantly in the period of high metabolic. During of high metabolic, there appears obvious IR, but the mechanism is still unknown. Therefore, it is urgent to solve this problem. The research of IR on diabetes reveals that TNF-α plays a important role in insulin resistance and insulin secretion. TNF-α and FFA affect many links of the signals of insulin pathways, which is the key influence for IR’s appearance. IRS-1and GLUT4are the key signal molecular for insulin signals pathways, and each is abnormal can lead to the IR abnormality. Objectives:The features changes of the serious burning in the process of high metabolic, that’s to say, the abnormal and increasing TNF-α and FFA are as the entry points, and the serious burning rat is as the model, which is to research the influence of IRS-1and GLUT4between these two sides, to clarify the mechanism of IR after burning seriously in the period of high metabolic, to provide experimental evidence of insulin resistance for science and effective control, and to have the excellent clinical value for high blood sugar control for serious burning and trauma.Methods:1.Classifying randomly48male adult SD rats into experimental group and comparison group, that is burning group and false burning group; after anesthesia, the32rats of burning group shave their backs to sharp a burning area, then put them into100℃boiling water foe12seconds to make the30%TBSAIII°; after serious burning, injecting balance liquid5ml into intrapersonal to make the fluid resuscitation, and putting the iodophor on the wounds to clear the wounds and to keep the scab; putting the16rats of false burning group into20℃warm water for seconds, and collecting blood from their hearts after anesthesia, respectively after1h,24h,48h,72h,120h,168h and336h to use automatic blood glucose meter to determine glucose; using rat insulin ELISA reagent box to determine density of insulin in plasma after centrifugal blood; calculating insulin resistance index in steady state pattern, analyzing the changes of insulin resistance and insulin secretion, and finally to determine synthetically the resistance state of insulin.2.The analysis of the relationship among TNF-α,FFA and insulin resistance: collecting blood samples after1h,24h,48h,72h,120h,168h and336h, using LISA reagent box to determine the level of TNF-α and FFA, and analyzing the relationship between these two sides and insulin resistance.Results:1.Using serious burning rats SD rats model is to have a research, and finding out that insulin serious resistance exists in the period of high metabolic. It researches that there are obvious diversities in statistics between glucose, insulin density and HOMA-IR in the period of high metabolic and the comparison group.2.Comp arising with the false burning group, the TNF-α and FFA both increase in the serum of the burning group. The GLUT4level in muscle organize of the burning group is obvious different from the false burning group, and the IRS-1level in muscle organize of the burning group is obvious different from the false burning group.Conclusions:1. In experiments on live animals, TNF-α and FFA play important role in insulin resistance mechanism of high metabolic period of serious burns.2. Serum TNF-α and FFA level are elevated after insulin resistance mechanism of high metabolic period of serious burns. Consequently, TNF-α and FFA play important role to insulin resistance through channels insulin post-receptor signal transduction. The TNF-α and FFA transform were significantly positive correlation, so TNF-α function is relatived to FFA in insulin resistance.3.In experiments on live animals, the TNF-α in myocyte directly or indirectly restrain the activation of insulin receptor, participate in regulation function of insulin receptor. IRS-1of high metabolic period of serious burns affects insulin resistance mechanism and ser-307phosphorylation is significant rise, tyrosine phosphorylation role is significantly lower in osculation.
