Objective: Investigate the clinical application of Catheter–directed thrombolysison acute deep vein thrombosisMethods: selected Paitents that diagnosed with acute deep venous thrombosis,divided them into three groups randomly. group A: discontinuous thrombolysis bythe superficial dorsal vein of foot, group B: Sustained thrombolysis by the superficialdorsal vein of foot, group C:Sustained thrombolysis via the small saphenous veinwith catheter. each group were treated with thrombolytic therapy of urokinase60×10.4IU/day.observed the thigh and calf circumference difference、 venouspatency score、 urokinase dosage, inspect Its complications, calculate the limbswelling rate and venous patency rate,then analysis the differences in efficacybetween the three groups.Results:1:All patients in the limb swelling and pain degrees reduced to someextent after the treatment.2: Pre-treatment groups in age, disease duration, skintemperature difference, the plasma D-dimer, and thigh circumference difference、calf circumference difference,venous patency score was no significant differenceamong the three groups (P>0.05),(Table1).3: after the treatment group Ccompared with group A and B: limb swelling rate is higher (80.00%)、 venouspatency rate is biger(90.00%)、venous patency score is lower (0.65)、urokinasedosage is lesser(190×10^4IU)、treatment time is shorter (4.45days)、thigh andcalf circumference difference is smaller (1.00CM,0.75cm), by comparisonthere are significant differences (P <0.01).4: after the treatment group B comparedwith group A: Limb swelling rate is higher (64.00%)、venous patency rate is biger(62.00%)、venous patency score is lower (3.05), urokinase dorsage is lesser (529×10^4IU)、treatment time is shorter (8.90days)、thigh circumference difference issmaller (1.80CM), by comparison there were significant differences (P <0.01),while calf circumference difference was no significant difference (P=0.685).5:Among the three groups there are no significant difference on their Complications (P=0.20). Conclusion: with catheter-directed thrombolysis Via Small saphenous vein inacute deep vein thrombosis there are higher limb swelling and venous patency rate,lower venous patency score, less urokinase dosage、shorter treatment time, smallercircumference difference in thigh and calf. It is superior to the discontinuousthrombolysis by the superficial dorsal vein of foot group and Sustained thrombolysisby the superficial dorsal vein of foot group. It dose not increase the risk of bleeding. Itis a safe and effective method.
