Effects Of Qing Yi Tang On Gastric Emptying And Intestinal Impelling Rate In Rats With Acute Necrotizing Pancreatitis

Objective observing experimental rats’gastric emptying rate,intestinal impelling rate, and histopathological grading for pancreas and small intestine, after Qing YiTang were perfused into the stomach of rats with acute necrotizing pancreatitis (ANP). To study the effect of Qing Yi Tang on ANP rats’gastrointestinal motility.Methods60SD rats were (sortition) randomly divided into three groups:ANP (N)group (N=20), Qing Yi Tang(Q) group (N=20), sham operation(S) group(N=20). Every groups ahead were randomly divided into2period:the Q1(n=10)、Q2(n=10); N1(n=10)、N2(n=10); S1(n=10)、S2(n=10).All rats in N group and Q group were injected5%sodium taurocholate (0.lml/100g) into the pancreatic capsule after opening abdominal to set up ANP model, whlie the duodenum were turned in S group only. After modeling and closing abdomen, Q1group were perfused Qing Yi Tang (lml/100g) into stomach in2hours and10hours respectively, Q2group were perfused Qing Yi Tang (lml/100g) into stomach in6hours and14hours respectively, S group and N group were subcutaneously perfused normal sodium (lml/100g)into stomach.Three groups rats were perfused blue dextran (0.4ml) into stomach after24h. The three groups were openned to the abdomen again after30minutes to observe ascites, saponification spot and the alteration of pancreas and gastrointestinal in general, taking measuring the concentration of AMY, and Ca2+in portal vein blood sampling, the tissue of the pancreas and intestines were observed through light microscopy after HE staining and the pathological score were evaluated. Results:1、Celiac general situation:the N group were seen to faint red hemorrhagic ascites, some saponified spots, the flaky pancreatic hemorrhage and necrosis, Gastric bowel bilges gas obviously, congestion and edema of intestinal wall obviously. The Q group were seen to the light hemorrhagic ascites, the saponification punctate spots, the flaky pancreatic point of hemorrhage and the necrosis,bilges gas and expansion of intestines and stomach congestion and edema of intestinal were lighter than N group. Such changes that mented above could not be observed in S group.2、Detecting on the concentration of AMY in the serum of the blood:There was significant difference between AMY concentration of N group and Q group (P<0.05), the concentration of AMY in N group and Q group were higher than S group (P<0.01).In the three groups of the different time points, no significant difference was seen in the concentration of the AMY in the serum (P>0.05)3、Detecting on the concentration of Ca2+in the serum of the blood:There was significant difference between Ca2+concentration of N group and Q group (P<0.05), the concentration of Ca2+in Q group and N group were higher than S group (P<0.01).The concentration of the Ca2+in the serum in the Q group of the different time points, compared Q1group with Q2group, Q1group higher than Q2group,(P<0.05). The concentrations of Ca2+in the serum in the N group and S group of the different time point group:there is no significant difference, P>0.05.4、Gastric emptying rate and intestinal impelling rate:significant difference were showed in gastric emptying rate and intestinal impelling rate between N group, Q group and S group (P<0.05). The gastric emptying rate and intestinal impelling rate in Q group were higher than N group(P<0.01).The gastric emptying rate and intestinal impelling rate in Q group and N group were higher than S group (P<0.01). Compared with gastric emptying rate and intestinal impelling rate between the different time points in the Q group: compared Q1group with Q2group, Q1group higher than Q2group,(P<0.05).Compared with gastric emptying rate and intestinal impelling rate between the different time point group in the N group and S group, there is no significant difference, P>0.05. 5、Pancreas histopathology:①Q group:Q1group:edema and pieces of hemorrhage can be seen in pancreas, inflammatory cells infiltrated.②Q2group: obvious edema and pieces of hemorrhage and necrosis can be seen in acinar cells of pancreas, a number of inflammatory cells infiltrated, some of the normal gland structure damaged.③N group:acinar cells broken down, collapsed, broad areas of hemorrhage and necrosis in pancreas, a large amount of inflammatory cells infiltrated, most normal gland structure of pancreas disappeared.④S group:no necrosis and hemorrhage,no inflammatory cells infiltrated,the gland structure of the pancreas is normal,.6、Intestinal histopathology:①Q1group:edema can be seen in intestinal villis, the intestine subepithelial space amplified, inflammatory cells infiltration;②Q2group:some top epithelial cells slough, intestinal villis disorderly arranged, edema can be seen in interstitial vilis, the intestine subepithelial space amplified, a number of inflammatory cells infiltration;③N:a large number of top epithelial cells slough, intestinal villis disorderly arranged, obviously edema can be seen in interstitial, the intestine subepithelial space amplified, a plenty of inflammatory cells infiltration;④S group:no any significant edema and significant inflammatory cells infiltration, intestinal villi structure was intact.7、The pathological score of the pancreas:significant difference were showed in the pathological score of the pancreas between N group, Q group and S group (P<0.05) The pathological score of the pancreas in Q group were higher than N group (P <0.01).The pathological score of the pancreas in Q group and N group were higher than S group (P<0.01).The pathological score of the pancreas in the Q group of the different time points:compared Q1group with Q2group, Q1group higher than Q2group,(P<0.05).The pathological score of the pancreas between the different time point group in the N group and S group, there is no significant difference, P>0.05.8、The pathological score of the small intestine:significant difference were showed in the pathological score of the small intestine between N group, Q group and S group (P<0.05). The pathological score of the small intestine in Q group were higher than N group (P<0.01).The pathological score of the small intestine in Q group and N group were higher than S group (P<0.01). The pathological score of the small intestine between the different time points in the Q group:compared Q1group with Q2group, Q1group higher than Q2group,(P<0.05).No significant difference can be found in the different time point group in the N group and S group on the pathological score of the small intestine, P>0.05.Conclusion:1、QYT can improve the the gastrointestinal motility of SAP rats.2、Earlier intervention with the QYT are superior than the late intervention.
