Objective By using0.02%mitomycin C in mechanical or laser subepithelialkeratomileusis (epipolis laser in-situ keratomileusis, Epi-LASIK) for treatment of highmyopia, investigate the impact of the corneal irritation, corneal epithelial healing time,uncorrected visual acuity, Haze and the tear film. Provide a reference for the clinicalsurgery of the eye refractive doctor.Methods Select fifty-seven high myopia patients(one hundred and fourteen eyes)for the Epi-LASIK in our hospital in July to October in2010. Randomly divided intotwo groups. Let use mitomycin group (MMC group) twenty-nine patients (fifty-eighteyes), did not use mitomycin group (control group) twenty-eight patients (fifty-sixeyes), patients were followed up for one year. Observe the corneal irritation, cornealepithelial healing time and visual acuity on the first day, the third day, the fifth day, theseventh day after surgery; Observe the Haze on the first month, the third month, thesixth month, the twelfth month after surgery; Observe the binocular visual acuity ofpatients, tear secretion, tear film breakup time, tear fern test and conjunctival impressioncytology before and on the first month, the third month, the sixth month, the twelfthmonth after surgery.Result1. Corneal irritation scores on the first day, the third day, the fifth day, theseventh day after surgery, t of two sets were-0.19,0.00,-1.00,1.00, respectively,P>0.05, the difference was not statistically significant; Observe under the slit lampmicroscope in the early postoperative that the conjunctiva of the two groups had nosignificant congestive, corneal severe edema, scleral necrosis.2. Compare the Cornealepithelial healing time, the average time of MMC group was (5.17±1.03) ds, the averagetime of the control group was (5.11±1.00) ds, the difference was not statistically significant (t=0.00, P>0.05).3. Comparison of early postoperative uncorrected visualacuity, the difference was statistically significant on the first day after (t=-2.62, P<0.05);the difference was not statistically significant on the third day, the fifth day, the seventhday after surgery (t=-1.61, t=-1.80, t=-1.83, P>0.05).4. Two groups of visual acuitycompared (t=0.73, t=0.43, t=-0.74, t=0.96, P>0.05), the difference was not statisticallysignificant on the first month, the third month, the sixth month, the twelfth month aftersurgery.5. Compare the formation of Haze, the difference was statistically significanton the first month, the third month, the sixth month after surgery,(χ2=12.43, χ2=10.94,χ2=11.51, P<0.05); the difference of Haze was not statistically significant on the twelfthmonth after surgery,(χ2=0.46, P>0.05).6. The difference of tear secretion, tear filmbreakup time, tear fern test and impression cytology examination with preoperative ofthe two groups was not statistically significant before surgery and on the first month, thethird month, the sixth month, the twelfth month after, P>0.05; the tear secretion, tearfilm breakup time, tear fern test were compared with the preoperative difference wasstatistically significant on the first month, the third month after, P<0.05; After the sixthmonth, the twelfth month, the difference of tear secretion, tear film breakup time, tearfern test were compared with preoperative was not statistically significant, P>0.05.After the first month, the third month, the sixth month, the difference of impressioncytology examination with preoperative was statistically significant, P<0.05; comparedthe imprinting cytology between the two groups was no significant difference on thetwelfth month after, P>0.05.Conclusions Application of0.02%mitomycin C in Epi-LASIK surgery fortreatment of high myopia that effected in the cutting zone, application time controlled in20-30s, flushing volume less than200ml that can effectively inhibit the formation ofHaze, and no obvious side effects, did not aggravate the corneal irritation and tear filmdamage, had no significant effect on corneal epithelial healing time and earlypostoperative UCVA recovery and conducived to the stability of the post–vision.
