| Purpose (1) To observe the effect of parenteral nutritional support on liver function,nutritional status, lipid metabolism, immune function, complications and its security andavailability in patients with cirrhotic portal hypertention after pericardialdevascularization.(2)To observe the effect of alanine-glutamine parenteral nutrition onnutritional status, liver function, immune function, inflammatory responses,complications, security and availability in patients undergoing portal hypertentionsurgery.Methods (1)55cases after pericardial devascularization were randomly assigned into2groups: PN group receiving parenteral nutrition support with structured triglycerides,nitrogen, glucose, vitamin in regular “3liter bag†from day1to day7after surgery. andControl group receiving regular fluid supplement, with the same nitrogen, glucose,vitamin as PN group. Check postoperative liver function, lipid metabolism, immunefunction, body composition and complications on day1, day3, day5, day7after thesurgery.(2)40cases undergoing portal hypertention surgery were randomly assignedinto2groups: Control group(N=20) receive parenteral nutrition support with structuredtriglycerides, glucose, nitrogen, vitamin from day1to day7after surgery. Study group(N=20) receive alanine-glutamine parenteral nutrition. Check the nutritional status, liverfunction, immune function, inflammatory responses, body composition andcomplications on day1, day3, day5, day7after the surgery.Results (1) There were significant differences between the two groups in liver function,prealbumin, body compositon(P<0.05), There were no significant differences between the two groups in lipid metabolism, immunity and complications(P>0.05).(2) Therewere significant differences between the two groups in arm musclecircumference(AMC), prealbumin, Retinol binding protein, CD4~+Tcells, CD8~+Tcells,IgG, IgM, CRP.and the recovery time of intestines fuction(P<0.05). There were nosignificant differences between the two groups in liver function.(P>0.05).Conclusions (1) Structured triglycerides used in patients with cirrhotic portalhypertention after pericardial devascularization is well tolerated, safe and available, itcan accelerate the liver function, improve the nutritional status of the body.(2)Alanine-glutamine parenteral nutrition used in patients with cirrhotic portalhypertention after pericardial devascularization can improve the nutrition status and theimmunity of the body. It also can relieve the inflammatory responses, reduce thecomplications, accelerate the recovery of intestines function and it is well tolerated, safeand available. |
