Objectives1.To investigate the changes of the serum levels of Visfatin, Ghrelin,CA19-9and their associations to some metabolism parameters after different treatmentsof type2diabetes mellitus patients.2.To observe the effects of metformin and aerobic exercise on ADM of perirenaladipose tissue(PAT),epididymis adipose tissue(EAT) in obesity rats fed withhigh-fat-diet.Methods1.Ninty type2diabetic mellitus patients were treated for3months or7daysand divided into three groups.The subjects in A group received Acarbose for threemonths.The subjects in B group were treated with conventional subcutaneous insulininjection for multiple times for three months.The subjects in C group were treated withcontinuous subcutaneous insulin infusion for7days.The serum levels of fasting bloodglucose(FBG),2hour blood glucose(2hBG),triglyceride(TG),total cholesterol(TC),lowdensity lipoprotein(LDL),fasting insulin(FINS) and the serum levels of Visfatin,Ghrelin,CA19-9were measured before and after treatment.2.Seventy SD rats were randomly divided into normal diet group(ND) with10rats andhigh-fat-diet group(HFD) with the remaining60.The two groups were fed with normaldiet and high-fat-diet respectively.After16weeks, take40rats whose body weight weregreater than20%of the average body weight in ND group from HD group,the40ratswere randomly divided into four groups accepting intervention for6weeks: Fat group(F), Metformin group(M),swimming exercise group(S),the combination ofmetformin and swimming exercise group(MAS) with10rats in each group.ND group,Fgroup and S group were all treated with distilled water,M group rats were treated withmetformin (200mg/kg d),S group rats were assigned to swimming exercise,MAS grouprats were treated with the combination of metformin(200mg/kg d)and swimmingexercise.Body weight(BW), fasting blood glucose(FBG),triglyceride(TG),totalcholesterol(TC) and fasting insulin(FINS) of SD rats were measured before and afterthe intervention of6weeks. ADM protein expression levels in PAT and EAT weremeasured using immunohistochemical method.Results1.Changes of the Serum Levels of Visfatin,Ghrelin and CA19-9afterDifferent Treatments of Type2Diabetes Mellitus patients1.After treatment, Theserum Visfatin and CA19-9levels of three groups of patients were all decreasedsignificantly (P<0.05), and the serum Ghrelin levels increased significantly (P<0.05).2.The decrement of the serum Visfatin1evels (△visfatin) was positively correlated withwaist circumference(WC),WHR and the decrement of levels of TG, LDL-C, FBG.Thedecrement of the serum CA19-9levels were positively correlated with the decrement oflevels of TC(P<0.05),and the increment of the serum Ghrelin1evels were negativelycorrelated with the decrement of levels of TC(P<0.05).3.The decrement of the serumVisfatin, TG,TC, FBG and the increment of the serum Ghrelin in group C was more thanthose in group A and (or) group B.2.Effects of Metformin and Aerobic Exercise on ADM Protein Expression Levels inDifferent Tissues of Obesity Rats1.After16weeks of high-fat-diet, BW, FBG, TG,TC and FINS,HOMA-IR levels of each group in obesity model were higher than thosein ND group with a statistical significance (P <0.05).2.After intervention of6weeks,BW, FBG, TG, TC and FINS,HOMA-IR levels of the group M, S and MAS ratswere decreased compared to F group rats(P <0.05).3.In group M, S and MAS, there is no statistical difference of BW, FBG, TG, TC and FINS,HOMA-IR.4. ADM proteinexpression of group F in PAT and EAT is higher than group ND(P=0.008).5.ADMprotein expression of group M, S and MAS is lower than group F in PAT and EAT(P=0.027、0.001、0.000).6.BW, FBG, TG, TC and FINS,HOMA-IR levels of SD rats werenot related to ADM protein expression levels in PAT and EAT.Conclusions1. Acarbose, MDI and CSII can improve the glucose and lipidmetabolism disorder in patients with type2diabetes mellitus,reduce serum Visfatin andCA19-9levels, increase serum Ghrelin level.2.Metformin, aerobic exercise, the combination of metformin and aerobic exercise canimprove the glucose and lipid metabolism disorder, insulin resistance of rats fed withhigh-fat-diet, and can down-regulate ADM protein expression levels in PAT and EAT.
