The Clinical Observation Of Early Application Nourishing Yin Mixture Prevent And Treat The Side Effect Caused By Large Dose Hormone Therapy In Nephrotic Syndrome

Objective:To Formula the best delivery time of Nourishing Yin mixture in preventing and treating the side effect caused by large dose hormone therapy in the treatment of nephrotic syndromeMethods:(1) All patients came from the Wuhan NO.1hospital from May2010to February2012in the nephrosis internal medicine department outpatient service and in hospital case, and these cases were diagnosed with primary nephrosis syndrome. Acording to the inclusion criteria and exclusion criteria,60cases met the study criteria, And50examples had the kidney pathology result material, Minimal Change Disease (MCD) were15examples, the (MsPGN) Mesangial Proliferative Glomerulonephritis10examples, the Membranes Nephrosis (MN)12example, Focal Segmental Glomerularsclerosis (FSGS)7xamples, Mesangioca illary Glomerulonephritis (MPGN)6examples.(2) All cases were randomly divided into the treatment group I, treatment group II and control group. Control group were given standard hormone therapy that is widely used stages of prednisone 1.0mg/(kg·d) Symptomatic and supportive therapy (including diuretic, lower blood pressure, lipid-lowering, anticoagulant, anti-infection, etc.), on the basis of the control group the treatment group Ⅰ was given Nourishing-Yin mixture(Zhimu, Cortex Phellodendri, raw land, dogwood, yam, figwort, Dendrobium, Polygala, tuckahoe, tortoise plastron, Atractylodes, Radix Paeoniae Alba, Radix Ophiopogonis, orange) in first days, and treatment group Ⅱ was given at fourth weeks later.(3) Observation time:8weeks.(4) Observed in patients before treatment, after2week,4weeks,6weeks,8weeks of clinical symptoms and signs, hepatorenal function,24-hour urinary protein, serum albumin、cholesterol、 triglycerides, Tradition Chinese Medicine symptom score of Hyperactivity of fire due to yin deficiency, Western medicine complications frequency(5)Data using statistical package deal SPSS17.0Results:(1) Hyperactivity of fire due to yin deficiency syndrome of TCM integral comparison:before the treatment integrals of each group had no difference, after8weeks of treatment, the treatment group Ⅰ integral at least, group Ⅱ were lower than the control group, and each group had a statistically significant difference between uniform (P<0.05)(2)Western medicine complications frequency comparison:in Cushing syndrome, hypertension, hyperglycemia, infection, deep venous thrombosis, acne, menstrual disorders, gastrointestinal symptoms, osteoporosis and osteonecrosis9complications of cumulative frequency, treatment groupⅠ<Ⅱ<control group. Amoog treatment group Ⅰ and Ⅱ and the control groups, the differences were statistically significant (P<0.05), the treatment group Ⅱ and control group had no significant difference (P>0.05).(3) Biochemical index:groups before treatment,24h urine protein quantitative lipid analysis and hepatorenal function, have no difference (P>0.05).8weeks after treatment:the biochemical indexes of group I:are obviously superior to the group Ⅱ and the control group. In UP, ALB, TC, TG results, the difference was significant (P<0.05); In UP, ALB, TC the treatment group Ⅱ and control group had significant differences (P<0.05), TG no statistical difference (P>0.05)Conclusion:(1) NourishingYin mixture could control the side effects caused by large dose hormone therapy in nephrotic syndrome, and the Curative effect had significant correlation of the dosing time of Yangyin mixture, delivery time sooner, hormone, the side reaction more less ang light.(2)In the nephrotic syndrome with large dose hormone therapy phase, nourishingYin mixture can obviously reduce proteinuria, increased serum albumin.
