ObjectiveThis study aimed to investigate the bronchial asthma in patients with fraction of exhaled nitric oxide(FeNO)、serum IgE and lung function(FEV1%) levels, observe its significance and relevance, to explore its significance of diagnosis and treatment in patients of bronchial asthma. This study investigated the changes of the fraction of exhaled nitric oxide(FeNO)、serum IgE and lung function(FEV1%) levels in non-acute exacerbation of bronchus asthma patients with using inhaled corticosteroids before and after treatment,to explore the influence and the clinical significance in FeNO of inhaled corticosteroids standardized treatment or not.Methods1.object:Select100cases test the levels of FeNO by the NO analyzer (NIXO. Aerocrine AB; Soina, Swede), fasting serum total IgE,and one second forced expiratory volume is expected to value percentage (FEV1%),34cases of acute exacerbation,36cases of remission,30normal healthy controls were measured the levels of FeNO、serum total IgE and forced expiratory volume in one second to predicted values percentage (FEV1%) for statistical analysis.100cases of78who have hospital-diagnosed patients with bronchial asthma,including36cases of acute exacerbation,42cases of non-acute exacerbation of bronchial asthma patients,the non-acute exacerbation of bronchial asthma patients according to whether the standardized application inhaled corticosteroids (ICS) treatment is divided into the hormone therapy group and the non-hormone treated group. The normal control group of22cases without heart and lung diseases、allergic diseases infection history and application of medication history in recently were normal healthy people.2. Method:All of the subjects are extracted fasting blood samples (4ml/person) in the morning, uses the enzyme league immune method (ELISA), Determination of serum total IgE and peripheral record numerical, the NO measuring instrument is provide by Beijing BaiSai Joint Science and Technology Development Co., Ltd. In strict accordance with the operation of the product, the requirement in50ml flow control/s, test results to FeNO PPB (Parts Perbillion) as a unit, asked subjects to intrapulmonary gas to exhale, the lips tightly sucking device, forced inspiratory5s, after intrapulmonary gas with steady speed discharge, lasts10s,90s after reading the numerical. Repeated testing lung function parameters3times, each time interval10minutes, take the best value, determination of FEV1%,and reading the numerical.Results1. Compare the level of FeNO、IgE and FEV1%of Bronchial asthma in acute exacerbation group and control group, statistical significant differences were existing(P<0.05).2. Asthma in acute and non-acute phase of the two groups of FeNO and FEV1%value was statistically significant (P<0.05), IgE is not statistically significant (P>0.05).3. Non-acute exacerbation of bronchial asthma patients group compared with the normal control group, FeNO level and serum total IgE value is still significant difference (P<0.05), but the FEV1%level but no significant differences (P>0.05).4. Observation of bronchial asthma in non-acute phase patients, not application of inhaled corticosteroids standardized treatment of FeNO the level was significantly higher than has been applied hormone treatment group, there was a significant difference (P<0.05), while serum IgE values are no significant difference (P>0.05).5. The study also found that changes in the diagnosed patients with bronchial asthma,FeNO level and peripheral serum total IgE content were positively correlated (r=0.266, p<0.05), and FeNO levels and FEV1%change was negatively correlated (r=-0.23, P<0.05).Conclusions1. These findings strongly indicated a role for FeNO in the eosinophilic airway inflammation, it can be used to evaluate bronchial asthma patients with airway inflammation in control and the degree of disease, dynamic detection provide meaningful reference index for clinical.2. Pulmonary function in FEV1%level is only in bronchial asthma in acute attack period are obviously different, but no specific change in the non-acute phase of bronchial asthma and normal healthy adult, indicating that FEV1%cannot be used as the evaluation of the degree of airway inflammation, lung function parameters in normal, airway inflammation is also the sustainable existence.3. After treatment with ICS FeNO is dreased, FeNO is able to evaluate the hormone sensitivity of bronchial asthma patients, and through its value changes to determine hormone dosage and course of treatment, and to avoid airway long-term inflammation leads to airway hyperresponsiveness, resulting in irreversible damage of the airway.
