ObjectivesBased on the summary of the practice of centralized purchasing in high value medicalconsumables, and the field survey about the purchasing and using of implantable cardiacpacemaker, this paper was aimed to analyze the key problems in the mode of purchasingand using of high value medical consumables, and to analyze the main factors whichinfluenced the doctors’ decisions on the choices about the use of the high value medicalconsumables.With above results, an indicator system used to define the concept of essential high valuemedical consumables was set up, which was expected to provide some advices for thegovernment to puts forward related decisions in the centralized bidding and purchasing ofessential high value medical consumables.MethodsLiterature review was carried out to collect information on the experience of centralizedpurchasing in high value medical consumables in China. Field survey and group discussionwere used to analyze the current situation and existing problems in the centralized biddingand purchasing and the using of the high value medical consumables, as well as the mainfactors which influence the selection of high value medical consumables. Delphi methodwas used to scan and adjust indicators and to weight indicators.Results(1)The supply and the use of high value medical consumables in the hospitals hadfollowing features: the number of varieties among different hospitals varied greatly, thespecific models in the different hospitals were always different, large proportion of medicalconsumables were import products, the purchase of some products had plenty of room toadjust.(2)In recent years, many measures related with the centralized bidding and purchasingof high value medical consumables had been in practice, which was an effective way of reducing the price of consumables. But there were some problems to solve: the selection ofessential high value medical consumables was ignored, which was very important to guiderational use of the consumables. And the proportion of imported products was too much toencourage the development of the national enterprise.(3)Based on the research about the key factors that influenced doctors’ choice of thespecific consumable models, an indicator system used to define the concept of essentialhigh value medical consumables was designed. After two round of Delphi expertconsultancy, we indentified an indicator system with4Level1indicators which includequality, economy, acceptance and supply indicators, and8Level2indicators which includeeffectiveness, safety, service life, price, convenient operation, commonly used, productionquantity and supply level. The weight of the8Level2indicators were0.354,0.179,0.100,0.171,0.067,0.061,0.034and0.034, respectively.SuggestionsFirstly, a uniform industry standard of medical consumable industry should be set up,which can help significantly in scientific management of medical consumables. Secondly,centralized bidding and purchasing of high value medical consumables should be promotedin the national scope. Finally, a scientific selection process of essential high value medicalconsumables oriented centralized bidding and purchasing should be established. |
