[Background and Objective]:To investigate the24h intraocular pressure (IOP), blood pressure (BP), andperfusion pressure (PP) of primary open-angle glaucoma (POAG) patients andcompare it with an age-matched normal group.[Methods]:IOP, SBP, and DBP were measured every2h in12healthy individuals and11POAG patients,measurements starting at10:00until08:00of the next morning. IOPmeasurements were made by a masked observer with a Tonopen-XL tonometer in insitting position from08:00to20:00and in supine position from22:00to06:00.Systolic and diastolic BP (SBP and DBP) measurements were performed with anautomated device.[Results]:There was no difference between two groups of age、gender; The POAG group hadthe higher IOP(P <0.001), there was no signifcant circadian change in IOP in bothgroups (P>0.05); IOP increased at night in both groups, in POAG patients IOP wassignifcantly higher at06:00, in normal group is at02:00; There was no difference inmean SBP, DBP, MAP (P>0.05) but was signifcantiy different for the minimum inboth groups(P<0.05), and the minimum was both at night, the pattern of circadianvariation in DBP (P=0.008), and MAP (P=0.015) was signifcantly different betweengroups but SBP(P=0.054) was not signifcantly different; The mean SOPP, DOPP,MOPP were significantiy higher in control group (P<0.05), and the circadian OPPs fluctuation were larger in POAG group.[Conclusion]: The POAG group had the lower OPPs and larger circadian fluctuation,which indicate insufficient ocular perfusion pressure was the risk factor of thedevelopment of POAG. As one of the vascular risk factors of development of POAG,OPP reflects the vascular status at the optic disc, it may be more relevant thansystemic BP alone.
