Objective:Examine the impact of the p-gp expression level in tumor-associated fibroblast of non-small cell lung cancer to the recurrence or distant metastasis of the NSCLC patients postoperative within one year. Study the preliminary mechanism of the TAF of NSCLC evade the immune and Drug-Resistance.Methods:The P-gp in TAF of non-small cell lung cancer tissue and normal lung tissue were detected by using Max-VisionTM immunohistochemical method, to observe the p-gp expression level in TAF of NSCLC lung cancer tissue and normal lung tissue, and follow up the postoperative patients within one year, to find out the relation between the expression level with the early period of recurrence or distant metastasis.Results:The p-gp expression level in the TAF of NSCLC lung cancer tissue is higher than the normal lung tissue (P<0.01). The positive rate of P-gp expression in TAF of48NSCLC patients’lung cancer tissues was70.8%. The expression level of P-gp in TAF associated with the patients’ age, degree of tumor differentiation, clinical stage, with or without lymph node metastasis. The positive rate of P-gp in the TAF with the age≤45years patients is higher than the age>45years patients (P<0.05). The positive rate of p-gp in the TAF of the patients with poorly differentiated tumor was higher than the well-differentiated tumor patients (P<0.05). The positive rate of P-gp in the TAF of the well-differentiated and middle-differentiated patients’ was significantly lower than the poorly-differentiated patients’(P<0.01); The positive rate of P-gp in the TAF in patients of clinical stage Ⅲ was higher than those of clinical stage I (P<0.05), and positive expression rate of the P-gp in the TAF of clinical stage Ⅱ was not statistically different compared both with stage I or stage Ⅲ; The positive rate of P-gp in the TAF was higher in patients with lymph node metastasis than those in ones without metastasis (P <0.05). The expressions level of P-gp in TAF have no-associated with the gender, the tumor size, the types of histology (Squamous cell carcinoma and Adenocarcinoma)(P>0.05). The postoperative patients with p-gp strongly positive expression in the TAF have higher rate of recurrence or distant metastasis than the negative or weakly positive expression in the TAF of NSCLC within one year (P<0.05)Conclusion:The positive express of P-gp in TAF play an important role in the occurrence and development of NSCLC. The positive express of p-gp in the TAF of NSCLC patients promote the formation of tumor drug resistance. The TAF and P-gp co-expression protect the lung cancer cell together and be beneficial of NSCLC recurrence or distant metastasis in the early period of postoperative patients, but the mechanisms still need further study. The detection of the P-gp expression level in TAF of NSCLC can help us to make the best choice of the personality therapy and personality drug, which also can help us to judge the prognosis of NSCLC patients.
