Diabetes mellitus is a major epidemic of this century,which has increased inincidence by50%over the past10years. The World Health Organization states that347million people worldwide were suffering from diabetes in2008, which equates to9.5%of the adult population. The incidence of diabetes is rapidly increasing withestimations suggesting that this number will almost double by2030. Diabetic vascularcomplications also increases with the incidence of diabetes is increasing year by year,Diabetes mellitus is a chronic illness characterised by increased risk of macrovascularand microvascular complications,and cardiovascular disease (CVD) is the primarycause of mortality in patients with diabetes. Microvascular complications are a majorcause of morbidity in diabetes populations and have been associated with heightenedrisk of systemic vascular complications. As early as not found in time, development toa certain extent resulting in patients with an important cause of disability to death. Theidentification of markers of macrovascular and microvascular disease could providenew information about the pathogenesis and early diagnosis of diabetes complicationsand facilitate decision-making in terms of prevention and treatment. Red blood celldistribution width (RDW) is a measure of the variability in size of circulatingerythrocytes (anisocytosis) and is routinely assessed in the evaluation of anaemia.Increased RDW levels are related to impaired erythropoiesis or erythrocytedegradation,which may reflect the high inflammation and oxidative stress states.Although higher RDW levels have been related to cardiovascular morbidity andmortality in several study populations, RDW has not been studied as a marker ofcomplications in diabetes. Threefore, this paper mainly discusses the changes in thediabetic vascular complications and the predictive role of RDW.Objective:To investigate the changes and value of Red blood cell Parametersin the diabetic vascular complications.Method:The433cases of T2DM patients were collected from Sept.2012to Mar.2013in our hospital. According to whether or not with vascular lesions, dividedinto simple T2DM (I) and diabetic microangiopthy (II), diabetic macroangiopthy(III),and the diabetic microangiopthy (II) were divided into diabetic retinopathy group (IIa)and diabetic nephropathy group (IIb) selected the same period in our hospitaldepartment of cardiology100cases simple coronary heart disease,select age-matched100healthy people as control group(HC), patients record their sex, age, BMI andother general information. Measurement of FBG, HbA1c, TG, TCHO and red cellparameters etc. The obtained data were statistically analyzed by SPSS14.0. SetP<0.05indicated the difference of statistically significance.Results:1、The result show that, in T2DM group,MCV、RDW were significantlyhigher than the healthy control group,the difference has statistical significance(P<0.05);RBC、 HGB、HCT、MCH、MCHC no statistical significance betweenHC group and T2DM group(P>0.05)。Compared with HC group, BMI、HbA1c、FPG、TG、TCHO、SBP、DBP were increased in the healthy,there is statisticalsignificance(P<0.05),and age was no difference(P>0.05)。2、Compared with I group,MCV、RDW were all increased between II and IIIgroup,there is statistical significance(P<0.05,P<0.01);Compared with II group,MCV was increased in III group,there is statistical significance(P<0.05),but there;In the III group, MCHC was lower I and II groups,there was statistical significance(P<0.01),RBC、HGB、HCT、MCH(P<0.05),among I、II、III groups, no statisticalsignificance(P>0.05)。Age、Duration、BMI、HbA1c、FPG、TG、TCHO was nostatistical significance in three group(sP>0.05),Compared with I group and II group,SBP、DBP were rised in III group,the difference has statistical significanc(eP<0.05)。3、RBC、HGB、HCT、MCH、MCHC、 MCV、RDW were no statisticalsignificance between III group and IVgroup(P>0.05);In the III group, BMI、FPG、TG、TCHO are higher than the IV group,there was statistical significance(P<0.05)。4、Compared with I、IIa,RDW was increased in the IIb group,the difference hasstatistical significance(P<0.05),but MCV、RBC、HGB、HCT、MCH、MCHC wereno statistical significance(P>0.05),There was no statistical significance between IIagroup and I group(P>0.05)。Compared with I、IIa,TCHO、SBP、DBP were increased in the IIb group,the difference has statistical significance(P<0.05)。Conclusion:Compared with healthy controls, patients with type2diabetesmellitus and complicated with vascular complication group MCV, RDW weresignificantly increased (P<0.05); MCV in diabetic DM with CVD was significantlyhigher than in microvascular;indicating:RDW and MCV can be used as a predicatorof CHD,and also can predict the risk of CHD with DM。Compared with the simplediabetic group, increased RDW associated with diabetic nephropathy(P<0.05), but nochanges in diabetic retinopathy group;indicating:RDW and MCV can not be used asa predicator of DR,but for DN,RDW may has certain prediction。...
