| General IntroductionApoplexy or stroke is one of the main diseases in today’s society,its morbidity is increasing year by year.Most of the patients who suffered from stroke left a variety of disabilities.Sleep disorder related with apoplexy often happen in the course of the disease.lt is reported that there are98%Post-apoplexy patients are troubled by sieep disorder problems, this proved that sleep disorders are the common concomitant symptoms.Patients with these problems have different clinical symptoms such as insomina,sleep delay-early syndrome,sleep dysrhythmia,sleepiness and so on.Perversion of sleep blongs to sleep dysrhythmia,it takes circandian sleep upside down, more sleep at day time,but lack of sleep at night,the total time of sleep is shorten as main manifestations.Serious sleep disorder not only bring bad influences to patients’life quality,physical and mental health,and the recovery of nerve function,but also aggravate illness as Hypertension,Diabetes mellitus,or even make cerebral hemorrhage and cerebral infarction happen again.Recent years,"biological clock" is getting international attention. The scientific researches confirmed nearly all physiology functions have the circadian rhythm."Biological clock" means the physiological functions, such as respiration, pulse, menstruation, hormone secretion, awaking and sleeping etc., have a periodic rhythm phenomenon. Awaking and sleeping, assembling the changes of day and night, manifests as a typical circadian rhythm and may lead to sleep disorder when the circadian rhythm is affected environmentally or artificially.To observe the clinical effects of acupuncture treatment by choosing acupuncture points according to the method of lingers in the heliospheric on patients suffer from Post-Apoplexy Insomnia-Somnolence Irregular Syndrome.Method and ContentClinical observations were made in the Nanfang Hospital between March,2010and July,2011.40patients met clinical diagnosis standard of cerebral apoplexy with sleep diserder problems were randomly divided into2groups according to the random digits table, All cases were gave usual treatments on the basis of western medicine including reduced patients’ blood pressure,given them brain protective agents and so on.On that basis,20cases (teratment group) were treated by choosing acupuncture points according to the method of lingers in the heliospheric with the guidinging principle of "Zhenjiudaquan" writed by Xufeng;another20cases (control group) were treated by tradtional acupuncture method, took Zhao hai,Shenmai,Shenmen,Yintang,Sishencong,Anmian as the main points.The2groups chose Neiguan,Shuigou,Sanyinjiao,Jiquan,Chize,Weizhong to treat the symptoms of stroke.Except for these masterpoints there were other matching acupoints such as Fenglong and Quchi with phlegm with heat and fuorgan constipation,Qihai and Guanyuan with qi-deficiency and blood-stasis type,Fengchi and Hegu with obstruction of collateral due to wind-phlegm,according to different TCM syndrome types.We chose disposable and sterile acupuncture needles(specifications:0.30×40mm) made from the equipment company in Suzhou,patients lie supine or sit there,disinfected the pionts with75%alchol,points chosen according to time will be operated first in teratment group.The patients should feel sour,numb,bloated or heavy sensation while acupuncture treatments were conducted,and then performed different strengthening or reducting acupuncture techniques according to the different physicals.Finally the needles were retained30min and manipulated one or two times.Press the points in case bleeding.The treatment was operated once a day,ten days for a course and lasted two courses.The sleep state degree,Athens insomnia scale degree and clinical nerve function damage degree of all cases were respectively marked before and after the treatment.The data collected was processed by SPSS13.0statistical software(SPSS13.0for windows).The measuring data was expressed by mean±standard deviations(x±s),and analyzed by T-test of independent sampler between two groups,Paired Sample T-test when compaired itself,the Chi-square test and Analysis of variance when the data belongs to categorical variable.Results1.The general information40cases were collected,including20patients with15men and5women from teratment group,whose ages range from41to79(58.5±2.53)years old,and the course of disease range from4to27(12.7±1.46)days,5cases were cerebral hemorrhage and15cases were cerebral infarction;The control group had20cases,including15men and5women, whose ages range from38to75(58.9±2.73)years old,and the course of disease range from4to26(13.0±1.53)days,6cases were cerebral hemorrhage and14cases were cerebral infarction.There was no statistically difference in aspects of age (F=0.169,P=0.683),sex (X2=0.000,P=1.000),the primary disease (F=0.189,P=0.666) and the course of disease (X2=0.476,P=0.490) between the2group cases analyzed by statistical test(P>0.05).2.Before the treatment the comparisons of sleep state degree (t=-1.132,P=0.265),Athens insomnia scale degree(t=0.727,P=0.472)and clinical nerve function damage degree(t=-0.959,P=0.343) between the2groups were no statistically differences.Before and after treatment,both of the2groups had statistically difference in aspects of sleep state degree(t=-6.681, P=0.000;t=-6.472,P=0.000),Athens insomnia scale degree(t=16.435,P=0.000;t=19.097,P=0.000) and clinical nerve function damage degree(t=11.74,P=0.000;t=11.78,P=0.000).It indicated that the two mehtods can improve the clinical symptoms of patients suffering from cerebral apoplexy with sleep diserder problems. Comparisons of the effects of sleep between the2groups after treatment had statistically difference(X2=6.290,P=0.043<0.05).There was also statistically difference in aspect of the level of inprovement of sleep state degree(t=4.074,P=0.000<0.01),combining with the significant difference in aspect of Athens insomnia scale degree and clinical nerve function damage degree between the2groups after treatment,we can say that the effecyively improvement of level of inprovement of sleep state degree may promote the rehabilitation of nerve function from a certain extent, the treatment group curative effect higher than those in the control group.Conclusion1.Both treatment group and control group showed good effects after treatment,patients’clinical symptoms were significant improved.2.After tretament the changes of Athens insomnia scale degree,sleep rate and clinical nerve function damage degree of treatment group were superior.3.There was statistically difference in aspect of the level of inprovement of sleep state degree(P=0.000<0.01),combining with the significant difference in aspect of Athens insomnia scale degree and clinical nerve function damage degree between the2groups after treatment(P=0.000<0.01),we may say that the effecyively improvement of level of inprovement of sleep state degree may promote the rehabilitation of nerve function from a certain extent.4.The acupuncture treatment by choosing points according to the method of lingers in Ithe heliospheric can improve clinical symptoms of patients suffering from Post-Apoplexy Insomnia-Somnolence Irregular Syndrome obviously. |
