| Objective:To explore level changes in serum matrix metalloproteinase2(MMP-2)ã€serum tissue inhibitor of metalloproteinase2(TIMP-2) and hypersensitive C-reactive protein (hs-CRP) in patients with type2diabetic nephropathy and to discuss their clinical significance,in order to peovide a theoretical basis for the arly prevention and treatment of diabetic nephropathy.Methods:According to urinary albumin excretion rate(UAER),90of patients with type2diabetes were divided into3groups, including30of normalalbuminuria group (UAER<20μg/min),30of micro-albuminuria group (UAER20-200μg/min), and30of macro-albuminuria group (UAER>200μg/min).Also30cases of healthy subjects who have undergone examination at the same time were chosen as control group. The serum MMP-2and TIMP-2was detected by means of enzyme linked immunosorbent assay(ELISA) and serum hs-CRP was analyzed by immunoturbidimetry, while simultaneously determining fast plasma glucose(FPG)ã€glycosylated hemoglobinAlc (HbA1c)〠triglyceride (TG)ã€total cholesterol (TC)ã€high density lipoprotein-cholesterol (HDL-C)ã€low density lipoprotion-cholesterol (LDL-C)ã€blood urea nitrogen (BUN)〠serum creatinine (Scr)ã€UAER and other clinical indexs,and observing the disparity in separate group and comparing.Results:the serum MMP-2of patients with type2diabetic nephropathy (including micro-albuminuria group and macro-albuminuria group) were significantly lower than that of patients merely suffering from type2diabetes and that of control group, and decreased with the increase of UAER in the four groups. Serum MMP-2level was significantly inversely correlated with duration of DMã€SBPã€FPGã€HbAlcã€TGã€TCã€LDL-Cã€BUNã€Scr and UAER levels, and significantly positively correlated with HDL-C(P<0.05).While serum TIMP-2and hs-CRP increased with the decrease of UAER, and they were significantly positively correlated with duration of DMã€SBPã€FPGã€HbAlc〠TGã€TCã€LDL-Cã€BUNã€Scr and UAER levels, and significantly inversely correlated with HDL-C(P<0.05). Besides, hs-CRP was significantly inversely correlated with MMP-2and positively correlated with TIMP-2(P<0.01).Conclusions:(1) With the increase of urinary albumin excretion rate, the level of MMP-2was down-regulated in patients with type2diabetes mellitus while that of TIMP-2and hs-CRP were up-regulation. Support that the abnormal expression of MMP-2ã€TIMP-2and hs-CRP may take part in the induction and development of the type2diabetes.(2) The level of hs-CRP was significantly inversely correlated with MMP-2while positively correlated with TIMP-2,support that MMP-2and TIMP-2have relation ship with inflammation. |
