ObjectiveTo investigate the immunohistochemical characteristic of salivary gland adenoid cystic carcinoma (SACC), and the prognosis significance of CD117expression of the adenoid cystic carcinoma.MethodsBy the technology of tissue microarray and immunohistochemistry, the expression of Smooth Muscle Actin(SMA), p63gene protein(p63), S-100Protein, Cytokeratin-pan(CK-pan), Epithelial Membrane Antigen(EMA), Ki-67Nuclear Antigen(Ki-67) and CD117/c-kit proto-oncogene(CD117) in41cases with SACC,19cases with salivary gland basal cell adenoma and31cases with salivary gland pleomorphic adenoma were observed. The expression intensity and distribution mode of the antibodies for above proteins were summarized and analyzed for their significance in diagnosis, differential diagnosis and prognosis of SACC, basal cell adenoma and pleomorphic adenoma.ResultsA different distribution characteristic of SMA, p63, S-100Protein, CK-pan and EMA was discovered among SACC, basal cell adenoma and pleomorphic adenoma. For the marker of SMA, p63and S-100Protein, it was positive in the peripheral zone of tumor cell, the lining cell of sieve pore and the outer cell of gland tubular structure in SACC; and in the peripheral zone of tumor cell and outer cell of gland tubular structure in basal cell adenoma; and diffused or nested arranged in pleomorphic adenoma. For the marker of CK-pan and EMA, it was strong positive in cytoplasm and cytomembrane of tumor cell in SACC; and also positive in cytoplasm of lining cell of the adenoid structure in basal cell adenoma; and also in tumor cells differentiated from the epithelial cell and partial myoepithelial cell of pleomorphic adenoma. For the marker of Ki-67, it was different between cases of SACC with different prognosis. There is statistical difference between the recurrent survival cases or dead cases and no recurrent survival cases (the P value of SNK-q test was7.4729and5.2186respectively, P<0.01). A strong positive expression of CD117was discovered in dead cases, and a statistical difference was found between them with the no recurrent survival cases (P=0.0394). There was a poor prognosis for cases with a high Ki-67index and a strong expressed CD117.ConclusionThe expression of SMA, p63, S-100, CKpan and EMA could be the valuble diagnosis mark for salivary adenoid cystic carcinoma, salivary basal cell adenoma and salivary pleomorphic adenoma.
