Like dental caries and periodontal diseases, malocclusion is a kind of common disease in Department of Stomatology. Defined as handicapping dentofacial anomaly, malocclusion caused by genetic factors and environmental factors doesn’t only affect functions like chewing and phonation, but also appearance and social activities. Thus mental disorders have been caused. With the development of society and the popularization of stomatology, more and more malocclusion patients, especially in adult patients, started to seek orthodontic treatment. Generally, the outcome of orthodontic treatment is not only about reasonable design and correct method, but also the cooperation of patients. The more cooperation, the better the outcomes. But the extend of patient’s cooperation in treatment is up against difficulties due to long treatment period for orthodontics, frequent re-visit cycle and various aching discomfort during treatment. Moreover, fixed appliance could have some social ramifications.This study focused on whether the psychological statuses of adult patients were associated with the degree of cooperation.Objective:Investigate psychological statuses of adult orthodontic patients with SCL-90and record the noncooperation behaviors in6months after orthodontic treatments are initiated to determine whether the mental health statuses of patients are related to the degree of cooperation. Method:92adult malocclusion patients (31males,91female, age from20-year-old to45-year-old), who visited for orthodontic treatment between December2011and December2012, were selected and determined with SCL-90to evaluate their mental health status. All the subjects met the following criteria:(I) No personal or family history of psychiatric disorders;(2) All the teeth had normal morphology and normal texture.Inspected and filed the record of the noncooperation behaviors in6months after orthodontic treatment was started. Here were5ways to assess the noncooperation behaviors:(1) Did not visit their orthodontists on time;(2) Broke the appliances like wires, brackets or bands;(3) Did not cooperate to use elastic loops or face-bows;(4) Lack of interest of different treatments;(5) Poor oral hygiene. All statistical analysis was performed using SPSS Statistics20software package.Result:1. Adult malocclusion patients scored significantly higher than the nation norm on the factors of interpersonal sensitivity, anxiety, schizophrenia and compulsion (P<0.05), higher on the factors of paranoid, phobia, depression and hostility (P>0.05) and lower on the factors of somatization (P>0.05). There was no significant difference between male and female patients (P>0.05). In this investigation,63.74%of the adult malocclusion patients had psychological problems. And we found that57.14%of them had at least one factor scored2or over,6.59%of them had at least one factor scored more than3. The top common psychological problems were interpersonal sensitivity, compulsion, anxiety, paranoid and schizophrenia.2. The score of noncooperation behaviors showed a scale of0to19, with an average of7.21±1.41points. There was no significant difference between male and female patients (P>0.05). Categorized the noncooperation behavior into3grades:(1) None or low:greater than or equal to0and less than5;(2) Medium:greater than or equal to5and less than11;(3) Serious:greater than or equal to11and less than19.3. There was a positive correlation between the noncooperation behavior and the factors of depression, anxiety, hostility and schizophrenia, and a negative correlation between the noncooperation behavior and the factors of interpersonal sensitivity, somatization and compulsion.Conclusion:The study confirmed that the psychological status of adult malocclusion patients were associated with the degree of cooperation. Having known well about the psychology of adult malocclusion patient before the orthodontic treatment is initiated, orthodontists can relieve psychological disorders in patients to improve cooperating quality of them.
