| Background:Surgical approach for cervical lymph nodes of the patients with oral squamous cell carcinoma in cN0stage is failing to reach a consensus. The argument focuses primarily on supraomohyoid neck dissection and radical neck dissection.Purpose:By applying meta-analysis, comprehensive quantitative analysis on the efficacy of two surgical approaches for cervical lymph nodes of the patients with oral squamous cell carcinoma in cN0stage can be given, so as to provide some reference on choosing appropriate surgical approach.Methods:We searched PubMed, Medline(Ovid), Elsevier ScienceDirect, Wiley online library, WANFANG databases, China Biology Medicine disc(CBMdisc), VIP databases and China National knowledge Infrastructure(CNKI) on line. Manual paper search and retrieve was not applied. Literatures were selected according to inclusion and exclusion criteria. After selection, quality evaluation was done and meta-analysis was processed by RevMan5.1which provided by International Cochrane Collaboration.Result:11studies were included based on inclusion criteria. The total sample size was1355cases, including SOND group593cases with246cases of recurrence or death, and RND group762cases with381cases of recurrence or death. By analysing the11studies, meta-analysis showed that no significant difference between SOND group and RND group for the patients with oral squamous cell carcinoma in cN0stage [RR=0.92,95%CI (0.83,1.03)] was observed. Sensitivity analysis indicated that there was either no significant difference between supraomohyoid neck dissection and radical neck dissection for the patients with oral squamous cell carcinoma in cN0stage, and the current study was observed to be quite stable.Conclusion:Present evidences revealed that two surgical approaches for cervical lymph nodes of the patients with oral squamous cell carcinoma in cN0stage did not differ significantly, besides both could provide favorable survival rate and neck control. Furthermore, complications of supraomohyoid neck dissection is fewer than complications of radical neck dissection, the former also improves the life quality of patients. However, more high-quality studies are needed to confirm the conclusion. |
