Objective: To evaluate the value of PET/CT in Solitary PulmonaryNodules.Methods:47patients (23males,24females) with Lung disease wereenorlled in this study ProsPectivel.They all received PET/CT test beforesurgey operated.All of the patients accord Solitary Pulmonary Nodules,Thencompared with the pathologic data. ROC curve was used to select the PETsemi-quantitative indicators(SUVmax), and compared with SUVmax=2.5;analysising the correlation between SUVmax=4.09and SUVmax=2.5;analysising the value in delay-imaging retention index (RI) of differentialdiagnosis of benign and malignant lesions; analysising the six CT signs inbenign and malignant lesions and analysising the difference of the occurrencerate; Comparisoning the diagnostic efficacy of the three diagnostic methods(PET, CT, PET/CT).Results:(1) Among the47patients with Solitary Pulmonary Nodules,34cases were lung cancer,the other13cases were benign lesions;(2) The ROCanalysis of SUVmax showed that the area under cure (AUC) was0.828, itwas statistically significant compared with0.5(P<0.001), the accuracy ofSUVmax diagnosis was medium; Youden index (YI) was4.09;(3) Thediagnostic value is higher in SUVmax=4.09as the cut-off-point (YI=0.63) than SUVmax=2.5as the cut-off-point (YI=0.43);(4)The SUVmax in noduleof maximum diameter≤2.0cm group is bigger than2.0cm<maximumdiameter≤3.0cm group (P<0.05);(5) The SUVmax in different nature oflesions(benign nodules and malignant nodules)(P<0.05), but the SUVmax indifferent types of pathology (squamous cell carcinoma and adenocarcinoma)were no statistically significant (P>0.05);(6) The delayed-imaging retentionindex (RI) of benign and malignant lesions was no statistically significant(P>0.05);(7) Of the6CT signs, spiculation, calcification levy, pleural tractionlevy leaf sign in emergence rate between benign and malignant weredifferent(P<0.05); vacuole sign, vessel convergence sign appeared in benignand malignant lesions showed no difference (P>0.05);(8)Of three diagnosticmethods (PET,CT, PET/CT), PET/CT in the diagnostic performance of thehighest (P<0.05).Conclusion (1)The optimal cut-off value of PET semi-quantitativeindicators (SUVmax) is4.09; the diagnostic value is higher thanSUVmax=2.5;(2)The nature and the size of the nodule impact SUVmax;(3)The diagnostic value of PET/CT is significantly higher than simply PET orCT; The dual-phase PET/CT can not improve the diagnostic performance ofSPN.
