OBJECTIVE:Carcinoma of the head of pancreas are malignant tumnors from the head of pancreas,accounting for about1%-2%of all the malignant tumors of human body. In those recent years, the incidence of pancreatic head cancer has significantly increased. Because the location of pancreas is deep,the early symptoms of this disease are not obvious, the surgery is so difficult and complicatons are so many, the diagnosis and treatment of pancreatic cancer has been the difficulty of General Surgery.This article aims to summarize our experience at the diagnosis and treatment of carcinoma of the head of the pancreas.METHODS:We restrospectively analyzed the clinical data and prognosis of103patients with carcinoma of the head of the pancreas treated in our department,from October2006to October2012,and found out the factors which influence the prognosis,and analyzed with stastistical methods.RESULTS:Among these103patients,57patients underwent radical pancreaticoduodenectomy, while34patients had palliative resection,12patients underwent laparotomy surgery. Median survival time of the three groups were28months-.9months、5months respectively. Significant difference was found between either two groups. According to the pathological results, we can divide the patients into two groups,patients lymph node metastasis and patients without lymph node metastasis.The5-year survival rate of the37patients with lymph node metastasis were0.00%,while the5-year survival rate of51patients with none lymph node metastasis were7.84%.By stastistical methods, we can conclude that there was significant difference (χ2=22.053, P<0.001) between the survival rates of the group with lymph nodes matastasis and the group without lymph nodes matastasis.The five year survival rate of group with nerve infiltration is2.1%,while the survival rate of the group without nerve infiltration is7.3%.There was none significant difference between the two groups according stastistical analysis χ2=3.072, P=0.082)。CONCLUSIONS:It is very important for those patients to diagnosis and be treated early,and the best choice of these patients is radical pancreaticoduodenectomy.Howerer,this is so big and complex a surgery that we need to fully assess the condition of the patient,and operate carefully in the operation and take care of the patient afer the surgery.By this we can reduce severe postoperative complications.
