Objective1. To examine the smoking and passive smoking status among national staffs inSuzhou city, their perceptions on tobacco control knowledge and their attitudes towardssmoking behavior and tobacco control measures, and to investigate the related factors ofsmoking behavior;2. Using the comprehensive measures of health education, administrativemanagement and mobile phone short-message service to intervene all the national staffsof participating units in Suzhou city, and evaluate the efficacy of the comprehensiveintervention.MethodCross-sectional study In May2011to September2011, we using census methodto investigate31public welfare services units, mainly including public service units,state organs, state-owned enterprises and institutions in Suzhou city. To understand thenational staffs’ smoking rates, quit smoking rates and passive smoking status and theirtobacco control knowledge and attitude, and using logistic regression analysisinfluencing factors with smoking behavior relations. The questionnaire was designed byEmory University, USA;Comprehensive tobacco control intervention trial All of the subjects of the31state functionaries in the cross-sectional study as the investigation subjects, usingcomprehensive intervention measures of health education, administrative managementand mobile phone short-message service to intervene all the respondents betweenSeptember2011to December2012. After intervention, a questionnaire survey was conduct to follow the intervention subjects’ smoking rates, related smoking factors andhealth knowledge awareness, attitudes towards tobacco control measures and behaviorsassociated with smoking.ResultsCross-sectional study results A total of2106subjects were investigated, totalsmoking rate was23.8%, the smoking rates for male and female were37.8%and1.1%,respectively.60.5%of respondents reported that their units were tobacco-free indoor;56.2%of respondents answered their units’ smoking regulations were comparativelythorough performed;17.8%of respondents think that their tobacco-free policies werevery effective in quitting smoking; In all of the respondents about the idea of quittingsmoking,31.7%respondents ready to quit smoking within12months; Lots ofrespondents reported personal health(43.3%), do role models for children(33.1%),forbidden of public places and workplaces(28.0%), affecting to non-smokers(26.4%),and their loved ones’ against(25.3%) could make them though of quitting smoking in avery great degree; Results of knowledge towards tobacco and secondhand smoking riskshowed: the respondents had a high awareness rates on Lung cancer (89.3%),Emphysema (76.7%), Yellow teeth (88.9%), Child lung diseases (81.7%) and Lungcancer (85.5%). But a poor awareness rates on Stroke (54.7%), Heart attack (55.6%),Erectile dysfunction (46.4%), Premature aging (67.9%) and adult heart disease (61.3%);Logistic analysis showed that women were a protection factor of smoking behavior (OR=0.03,95%CI0.02-0.05); the risk of smoking behavior increasing with the rising-age,reducing with the rising of cultural level, the55years old or more subjects had thehighest smoking behavior risk and the college/undergraduate and graduate had thelowest smoking behavior risk, the OR and95%CI were8.31(4.60-15.01),0.23(0.07-0.83) and0.24(0.07-0.86), respectively; participants who knew the hazard ofsmoking/secondhand smoking could reduce the occurrence of smoking behavior, theOR and95%CI were0.67(0.56-0.81)and0.66(0.55-0.80); The most worthmentioned in this study was that thoroughly executive carry out the smoking rules in thestate functionaries and visitors could reduce the occurrence of smoking behavior, theOR and95%CI were0.67(0.56-0.81) and0.66(0.55-0.80), respectively.Comprehensive tobacco control intervention trial results A total of2170subjects were investigated, the subjects’ total smoking rate was18.4%after comprehensive intervention, significantly lower than before (23.9%), the males’smoking rates reduced from38.1%to30.2%, the women’ smoking rates reduced from1.1%to0.6%. Compared with the baseline investigation data, after comprehensiveintervention, the national staffs’ awareness rates of tobacco and secondhand smokingwere significantly higher than before, all of the awareness rates were improved higherthan8%, such as the awareness rates of Strokes (78.6%), Heart attacks (77.9%), Erectiledysfunction (73.3%) and Premature aging (85.5%) and adult heart disease (77.3%). Andthe support rates of tobacco-free public places were significantly increased, the highestrate was the supporting of tobacco-free hotel room(87.5%); There were significantdifferences in the implementation rate of smoking regulations before and after theintervention (p<0.05), and the employees’ smoking rates of each project unit in theworkplace had dropped significantly(p<0.05), decreased from6.1%to3.6%; Moreover,the number of respondents who ready to quit smoking in12months or longer more thanbefore, rising from16.0%and34.6.0%before intervention to18.9%and35.8%afterthe intervention, respectively.ConclusionCross-sectional study (1) The smoking rate of Suzhou city-state functionaries washigh in china, knowledge of the specific effects of tobacco in respect of active andpassive smoking needs to be further improved;(2) The ratio of smoke-free indoorworkplace and the implementation rates of indoor tobacco regulations in some units wasvery low, so the aim of creating tobacco-free environment remains to be furtherimproved;(3) Smoking behavior related with gender, age, culture level, the knowledgeof smoking and whether executive smoking regulation and so on, improving the healthliteracy and strictly implementing the tobacco control regulations will be an effectiveway to reduce the smoking rates of Suzhou city-state functionaries.Comprehensive tobacco control intervention trial The effects of thecombination comprehensive intervention measures of health education, administrativemanagement and mobile phone short-message service was remarkable, mainly displayin:(1) The subjects’ smoking rates dropped significantly after comprehensiveintervention;(2) The implementation rates of tobacco control regulations in projectunits were significantly higher than before, and all this increased the indoortobacco-free places;(3) The national staffs’ awareness rates of tobacco and secondhand smoking were significantly higher than before, and also the subjects’ intention ofquitting smoking was improved after comprehensive intervention.
