Purpose: This paper is to evaluate the effects with different therapeuticmethods of developmental dislocation of the hip in children aged from12-18months.Material and method: A retrospective analysis of clinical data of125patients (aged from12to18months, mean15.1months), of whose150hips weredevelopmental dislocation of the hip, treated with conservative treatment andsurgical treatment from January1990to January2011. Ninety-nine of thepatients were female and Twenty-six were male. Twenty-five patients hadbilateral dislocation and one hundred were affected unilaterally.49hips wereoperated by open reduction and with Salter innominate osteotomy;24hips wereoperated by open reduction with Salter innominate osteotomy and Proximal femoralshortening rotation osteotomy.77hips were operated by conservative treatment.Results: All patients had been followed up for3years to5years and8months,50months in average. According to the modified McKay clinical criteria andSalter AVN diagnostic criteria. The excellent and good rate with modified McKaycriteria were in surgical treatment conservative treatment and conservativetreatment are91.89%and96.23%respectively for type Ⅱ, but avascular necrosisof the femoral head occuredin2hips (5.41%) in surgical treatment. There isno significant difference between two methods(P>0.05). The excellent and goodrate with modified McKay criteriawere in surgical treatment conservativetreatment and conservative treatment are91.67%and83.33%respectively for typeⅢ, but avascular necrosis of the femoral head occuredin3hips (12.5%) inconservative treatment. There is no significant difference between two methods(P>0.05).Conclusion: We believe the “walking age” is critical period to treatdevelopmental dislocation of the hip, it could make to yield twice the result with half the effort if Taking reasonable treatment. The conservative treatmentis better than the surgical treatment in type Ⅱ of developmental dislocationof the hip in children aged from12to18months. The surgical treatmentconservative treatment is better than the conservative treatment in type Ⅲ ofdevelopmental dislocation of the hip in children aged from12to18months.Besides, it is helpful both to functional recovery of the hip andprevention of AVN with replace to adjust the bracket of the outreach line withTCM Manual Traction and Massage.
