| Purpose:The study takes emergency physicians-who begin to receive specialist training after the first satge of residency training-as the research object, studies the growth law of emergency specialist, divides its growth stage and refines its stage characteristics. On this basis, combined with previous research, we will build the competency-based evaluation index in different stages of emergency physicians.Methods:(1) Using a literature study method to collect and collate the research literature ahout the growth law and competency of emergency specialist at home and abroad.(2) Using the expert interviews method to understand the growth law of emergency specialist in different stage (Preparatory interviews with8experts, formal interviews with15experts), and using the grounded theory interviews to summarize, analyze and classify the result of interview.(3) Using the Delphi method and survey method to solicit the experts’modifications and perfect views on the competency evaluation index system in different stages of emergency physician, a total of two rounds of survey (the first round of25experts, the second round of22experts),and computing experts’enthusiasm, the authority coefficient, expert opinions concentration and coefficient of variation, the expert coordination coefficient. The Excel table and SPSS16.0software were used for data entry and statistical processing.Results:(1) Classifying the status quo of literature about the growth law of f emergency physicians and competence at home and abroad, and comparing the competencies models of emergency physician in United States, Canada and Europe, the results of the comparison will be used as initial source of indicators in the evaluation system.(2) In accordance with the growth law of emergency physician, we have a reasonable division of emergency physician growth stage and its period:the3-years initial stage, the4or5-year intermediate stage and the advanced stage until retirement, and refine the growth characteristics corresponding to the stages.(3) On the basis above, we preliminarily build the primary, intermediate and advanced stages of emergency physician competency evaluation index system’s draft, and using Delphi method to modify the index system and its connotation. The results showed that the two rounds of questionnaire recovery rate is100%, which shows high motivation the experts have in this study; Two rounds of experts consulted by the authority coefficients were0.79and0.83, which shows the expert’s authority and reliability in the study; The majority arithmetic mean of index in the second round are higher than that in the first round, which illustrates experts’ increasing recognition and concentration on the indexs; As can be seen from the coefficient of variation, the diperison degree in the second round has declined compared to that in the first round, showing a improving convergence degeree. Expert coordination coefficient in the second round is higher than that in the first round, showing that coordination degree of the experts is on the rise, what’s more, significance test indicates that P<0.05,therefore, the results are statistically significant, and thus are desirable.Conclusions:The different stages of emergency physician competency evaluation index system consists of six core competency and its corresponding secondary indicators, there are six level indicators:patient services, knowledge and skills, communication, cooperation, management, education and research, professional qualities; The secondary indicators are settled according to the different stages of growth characteristics:15indicators of initial stage,16indicators of the intermediate stage,18indicators of the advanced stage. Owing to different importance we put in each capacity, the evaluation index system can effectively distinguish from emergency physicians at different growth stages, and the competency requirements for various stages of emergency physicians are different. For example:in the initial stage, the emergency physicians focus on emergency basic clinical thinking, basic knowledge and skills training, the emphasis is to develop the basic quality of emergency physicians and to become a qualified emergency physicians; In the intermediate stage, the emphasis is still to acquire the first aid knowledge and skills training, and begin to train the ability of management, research and teaching, the purpose is to train emergency physicians ability to work independently; In the advanced stage, focusing on the emergency physician’s comprehensive ability, and needing more prominent management and scientific research, the purpose is to allow emergency physicians to become a expert in the field. The evaluation index system has a strong practical significance:(1) According to the index system, emergency physicians can determine the growth stage itself and to understand their mastery of competency, and enhance the comprehensive strength of their own;(2) In accordance with the requirements of the evaluation index system, medical institutions and departments can better evaluate emergency physicians;(3) Evaluation index system covers the knowledge, skills, communication, management and professionalism, competence, which will provide a basis of emergency professional courses for future development, and is significantly important for regulating the training and assessment system of emergency physicians. |
