Yiqi Jiedu Only Strengthen The Clinical Research For The Treatment Of Primary Biliary Cirrhosis

Backgroud and Objective: Primary biliary cirrhosis (PBC) is a progressive cholestatic liver disease serologically characterized by the presence of high-titer antimitochondrial antibodies and, hitologically by chronic non-suppurative cholangitis and granulomata, gradually developed to liver fibrosis and cirrhosis. The etiology of the disease remains elusive, and pathogenesis is not fully clear. Ursodeoxycholic acid is the only drug that the U.S. Food and Drug Administration has approved its usage for primary biliary cirrhosis. It seemed to have a beneficial effect on liver biochemistry measures, but there is controversy on histological progression compared with the control group, and it is useless for symptoms. Some clinical severe manifestation (fatigue, pruritus and jaundice) and higher biochemical markers levels seem to predict a worse response to UDCA and a rapid progression of disease in PBC patients. There are little reports that Traditional Chinese Medicine is effective in some autoimmune liver disease and liver fibrosis. So, in this study we aim to observe the efficacy of yi qi jie du tong luo fang and UDCA in the treatment of46patients with primary biliary cirrhosis whose clinical stage were fibrosis or cirrhosis.Methods:All the patients hospital from September2011to February2013were diagnosised PBC with reference to the guidel ine suggested by American association for the study of liver diseases (AASLD) in2009. Select46patients whose clinical stage was fibrosis or cirrhosis and met all the following criteria were randomly divided into control group (n=25) and treatment group (n=21), treated with yi qi jie du tong luo fang and yi qi jie du tong luo fang combined UDCA respectively. All the drug granules of herbs in yi qi jie du tong luo fang were supplied by dong zhi men yi yuan pharmacy. At the same time, the base treatment was still needed. The clinical symptoms, biochemical markers, TCM syndrome scoring ultrasound and sideeffects were observed at0,3,6month, and the results would be estimate, statistical package for social science17.0to Statistical-Processing.Results:(1)The comprehensive effect after treatment:after6month treatment, the effect ive rate of pat ients in the treatment group is52.38%and total effeeti-veness is90.48%, while the control group is56%and92%. The comparison of curative effect between treatment group and control group had not obvious difference.(2)Clinical symptoms:The clinical symptoms as fatigue, pruritus, jaundice, were both improve in two groups after3and6months treatment. The theraPeutic effect after treatment for6months differed from that treatment for3months. The comparison between treatment group and control group had not obvious difference.(3) Hepatic function:after3and6months treatment, two groups’hepatic function as ALT、AST、GGT、ALP、TBIL has improved. Extending treatment to6months can increase the liver function improved. There was no significant difference between treatment group and control group.(4)Liver ultrasonic: after3and6months treatment, two groups’B-mode ul-trasonic improvement is not obvious.Conclusions:(1)yi qi jie du tong luo fang can release the symptoms, biochemistry of PBC patients whose stage in fibrosis or cirrhosis, but B-mode ultrasonic improvement is not obvious in6month time.(2) In6month, UDCA combined yi qi jie du tong luo fang in the treatment of PBC could not raise the effectiveness of yi qi jie du tong luo fang.(3)PBC is a progressing disease, the condition gradually worse, the correct maintentence treatment is needed, and had no definite course of treatment, so long-term study to evaluate the treatment of PBC is still needed.(4) Now, treatment of PBC is limited, research scattered; so large sample of clinical research is needed to find out the key Pathogenesis and targeted therapy.
