Objective: Through anterior approach and posterior approach operation for thetreatment of thoracolumbar fractures with spinal cord injuries.To the maximum extentrelieves pressure from the cord, restore stability of the spine and vertebral height, so as toform the spine biomechanics to restore the maximum.Observation on the therapeuticeffect of the anterior operation and posterior operation approach for the treatment ofthoracolumbar fractures with spinal cord injuries.Effect of anterior operation andposterior operation approach for the treatment of thoracolumbar spinal fracture withspinal cord injury had no difference, further study and obtained a spinal cord injury witheffective, reliable treatment of thoracolumbar spine fracture operation method.So thatpatients can get the best prognosis, reduce the psychological, physiological and economicburden of patients, has important significance for improving the quality of life of thepatients.At the same time, make an objective evaluation of the two kinds of operation.Methods: From Dec2010to Dec2012,60cases of thoracolumbar spinal fracturewith spinal cord injury who were treated in Department of orthopedics of YanAnUniversity Affiliated Hospital were collected.According to the ASIA grading standard,selecting the cases of B, C, D level injuries.These cases were randomly divided intoposterior approach surgery group(group A)and anterior approach surgery group(groupB).Two groups of patients Preoperative and postoperative5weeks index evaluationabout the condition of tactile sense score,movement score, cobb angle and the recover ofinjured spinal height. Through the preoperative, postoperative tactile sensescore,movement score, cobb angle and the recover of injured spinal height on theanalysis comparison between nterior approach surgery group and posterior approach surgery group. To observe the clinical curative effect of the difference of two groups ofpatients in preoperative and postoperative.To provide a reliable reference basis for theselection of Thoracolumbar spinal fracture with spinal cord injury clinical operationtreatment. And make statistics analysis,to evaluate the clinical value of these two surgerymethods.Results:There were no statistics differences of the age,sex ratio and damage degreebetween the two groups preoperatively. After the surgery,the condition of tactile sensescore,movement score, cobb angle and the recover of injured spinal height in posteriorapproach surgery group(group A) were better than the anterior approach surgerygroup(group B).There was significant difference between the two groups (P<0.05). Twogroups of patients had no obvious complications.Conclusions: In contrast,the anterior approach surgery can improve the spinal cordfunction. The internal fixation of operations’ mechanical performance is better and canobviously relieve patients’ anterior spinal cord compression Significant effect.Although there are arge trauma and bleeding in the operation, its effect is better than theposterior approach operation. At the same time,it is able to further improve the quality oflife of patients. |
