Research On The Relationship Between The TCM Constitution And Quality Of Life Of HBV Carriers

【Objective】1. To investigate the situation of Quality of Life and the distribution andcharacteristics of TCM constitution of HBV carriers.2. To study the effects of constitutional factors on the HBV carriers’ quality oflife.3. To provide clinical basis for establishing TCM health management mode forthe HBV carriers through the investigation and research, what is built forameliorating their quality of life, improving their general level of the health, reducingtheir infectiousness and preventing the further development of the disease.【Method】We made field investigations on HBV carriers who took physical examination incommunity hospitals and second-class hospital in the suburbs of Beijing. The surveycontent includes investigations of basic information, the TCM constitution of HBVcarriers using The Constitution in Chinese Medicine Questionnaire (CCMQ) and thehealth-related quality of life(QOL) using the medical outcomes study36-itemshort-form health survey (SF-36). We set up a control population who are non-carriersfor comparison purposes. The investigations all obtained informed consent. Theconstitution and the quality of life of the HBV carriers were analyzed statistically,comparing with the controls. Discuss the relationship between quality of Life (QOL)and the traditional Chinese constitution by Kruskal-Wallis Test and the LogisticRegression. 【Result】1. The constitutional types of the205samples from high to low rankings:Gentleness type(25.85%), Yang-deficiency type(13.66%), Wet-heat type(12.68%),Qi-deficiency type(10.73%), Qi-depression type(10.24%), Blood-stasis type(9.76%),Phlegm-wetness type(9.27%), Yin-deficiency type(5.85%) and Special diathesistype(1.95%). It had no significantly difference with the controls but the generalpopulation in Huabei (P<0.05), and there was aslo a significantly difference betweenthe controls and Huabei (P<0.05).2.67.32percent were multiple constitutions and the frequency of pathologicalconstitution ranked from high to low were Qi-deficiency type(31.71%), Qi-depressiontype(31.22%), Wet-heat type(30.73%), Yang-deficiency type(28.78%),Phlegm-wetness type(28.29%), Blood-stasis type(27.80%), Yin-deficiencytype(20.98%) and Special diathesis type(6.83%).3. Man and female carriers were significantly different in TCM constitutionaltype (P=0.00). With the increase of HBV carrier time, the percent of Gentleness typehad a tendency to decline from32.99%to19.54%while the pathological constitutionadd from67.01%to80.46%. To be specific, Yang-deficiency type added mostly,followed by Qi-deficiency type and Qi-depression type.4. The marks of carriers’ average and standard deviation of the PhysicalComponent Summary (PCS) domain and the Mental Component Summary (MCS)domain of SF-36were73.75±12.09and72.80±11.03. The HBV carriers’ quietly oflife was significant lower than the control group (P<0.05), except from the domainsof Role-Physical (RP), Bodily Pain (BP) and Vitality (VT).5. Different HBV carriers’ constitutions were in different score on the domainsof SF-36. In the domain of RP, Yang-deficiency type, Phlegm-wetness type, Blood-stasis type and Qi-depression type were lower than Gentleness type(P<0.01),and the Qi-depression type scored owest. In the domain of BP, comparing withGentleness type, the scores of Qi-deficiency type, Yang-deficiency type,Yin-deficiency type, Blood-stasis type and Qi-depression type were significantlylower(P<0.01), and the lowest on Yin-deficiency type. In the domain of GeneralHealth (GH), eight pathological constitution were significantly lower than Gentlenesstype (P<0.01), and the Qi-deficiency type scoredlowest. In PCS domain, also eightpathological constitution were significantly lower than Gentleness type (P<0.01), andthe lowest was Qi-deficiency type. In the domain of VT, Qi-deficiency type,Yang-deficiency type, Phlegm-wetness type, Blood-stasis type and Qi-depressiontype were lower than Gentleness type(P<0.01). In the domain of SocialFunctioning(SF), Yang-deficiency type, Yin-deficiency type, Phlegm-wetness type,Blood-stasis type and Qi-depression type were lower than Gentleness type(P<0.01).In the domain of Role Emotional(RE), Yang-deficiency type, Phlegm-wetness type,Qi-depression type and Special diathesis type were lower than Gentlenesstype(P<0.01). In the domain of Mental Health(MH), Qi-deficiency type,Yang-deficiency type, Phlegm-wetness type, Yin-deficiency type and Qi-depressiontype were lower than Gentleness type(P<0.01). In the MCS domain, expect fromSpecial diathesis type, other seven pathological constitutions were significantly lowerthan Gentleness type (P<0.05or P<0.01). And in all the mental health domains, theQi-depression type scored lowest.6. The Logistic Regression showed that the values OR of impaired QOL weresignificantly higher in eight pathological constitutions than Gentleness type. In thePCS domain, the values OR of Qi-deficiency type and Qi-depression type were thehighest, which were12.05and9.14times as much as Gentleness type. Specifically, the value OR of Blood-stasis type was the highest in PF and BP domains,Yang-deficiency type highest in RP domain and Qi-deficiency type highest in GHdomain. In all mental health domains (VT, SF, RE, MH, MCS), Qi-depression typewere the highest.【Conclusions】1. The characteristics of TCM constitutional type of HBV Carriers are follows:the proportion of pathological constitution is very large, about74percent.Yang-deficiency type, Wet-heat type, Qi-deficiency type and Qi-depression type arethe main pathological constitutions. The constitution types of HBV carriers arecomplex. Many people are multiple pathology constitutions which are often appearswith Qi-deficiency type, Qi-depression type and Wet-heat type. With the increase ofHBV carrier time, the percent of pathological constitution has a tendency to add.2. The chronic HBV carriers’ quality of life (QOL) has declined than the controlgroup, especially in the field of mental health.3. TCM constitution is an important factor influencing HBV carriers’ quality oflife. To be specific, Qi-deficiency type affects the Physical health greatly andQi-depression type is not only the main factor which impacts the mental healthmostly but also a potential impact for physical health. Improving the constitutions ofchronic HBV carriers is one of the most significant ways to enhance their quality oflife.
