ObjectivesTo evaluate the protective effect of Sivelestat sodium hydrate against kidney injury in ratswhich with severe acute pancreatitis by experiment operation.Observation of the changes ofAMY、NE、TNF-、Cr、BUN、IL-6in serum to explore the possible mechanism.The lever ofactivation of NF-kB in kidney tissues is also tested.Technical RouteTake72normal healthy male SD rats turned into three groups by completely randommethod: sham operation group (SO group), animal pathological models matched group (SAPgroup) and the treatment groups with Sivelestat sodium hydrate (SS group). Each group has24SD rats. Model SAP group was set up by5.0%sterile sodium(1mL/kg) injected retrograde ofpancreatic duct through punctures duodenum. In treatment groups, Sivelestat sodium hydrate30mg/kg(1ml solution made up with normal saline solution) was administered intravenouinjection by dorsal veins of penis after the establish create SAP models. The same volumenormal saline was intravenou injection after operation in the SAP group.Operation in abdomenwas made after anesthesia in SO Group, then turning the duodenum and pancreas, and venouinjection of equal volume of saline. Normal saline (30ml/kg) was injected subcutaneously inthree groups to compensate the water loss. Rats blood and pancreas, kidney tissue specimenswere taken for test at6,12,24hours with conduction. The activity of amylase (AMY), crea(Cr)and blood urea nitrogen(BUN) in serum were detected; the levels of neutrophil elastase(NE),tumor necrosis factor α(TNF-), interfilon-6(IL-6) in serum were measured by ELISA. Part ofpancreatic,kidney tissue were changed by HE staining and pathological severity scores of themwere assessed.Kidney tissues use antibody-marker means to survey the activity extention ofNF-κB. ConsequencesThe change of tests scores in SO Group is insignificant. There was no significant differenceamong each time points in each group (P>0.05). Serum AMY, Cr, BUN, NE, TNF-, and IL-6levels of SAP Group in the three time points were higher than that in SO Group (P<0.05).Pancreas, kidney pathological score and the lever of activation of NF-κB of kidney were higherthan the SO Group (P<0.05). Indices in SS Group campared with SAP group were improved atthe each time points (P<0.05). Pancreas and kidney pathological score was lower than that inSAP Group (P<0.05). The lever of activation of NF-κB of kidney was lower than that in SAPGroup (P<0.05).Conclusions1.NE, TNF-, IL-6are important parameter of inflammatory reactions in SAP. Theactivation of NF-κB is an important factor of SAP with kidney injury.2. Sivelestat sodiumhydrate can lower the degree of kidney injury in SAP. Its mechanism may be inhibiting therelease of serum inflammatory mediators such as NE, TNF-, IL-6, and restrained the activationof NF-κB in kidney tissue.
