Study On Health Service Need And Utilization Of Rual Inhabitants Of Three Counties In Fujian Province In2011

ObjectiveTo study the current situation of health service need and utilization of rualinhabitant in Jianyang, Yongding and Ninghua County in Fujian Province by fieldinvestigation for exploring the influence factors and exited problems, and providesome health policy reference and suggestions to allocate health resources morereasonably for making related policy decision and regional health planning of thegovernment.MethodsThe literature review was carried out analyze the policy environment of thehealth service demand and utilization. The questionnaire was conducted to investigatethe general situation of the families, the health service demand and utilizationa in15towns,30villages,600households,1899inhabitants. The data from the survey wereanalyzed by ACESS and SPSS18.0package.ResultsThe situation of health care access was great, and the new rural cooperativemedical system based basic medical insurances have covered most of the residents.The amount of the health service need increased slightly, chronic diseases became themajor problem of the residents, and the age was the main factor of affecting the twoweeks prevalence rate. The percentage of two week’s non-consultation of the patientsand the percentage of patients who should be but without hospitalization declinedapparently. The health service utilization has improved, and basically at townshiphealth centers, but the market share of township health centers was at lower level. Thereimbursement ratio was increased but still at low level. The coverage of essentialpublic health service has enlarged but the quality of the service should be enhanced. Policy RecommendationsTo strengthen the integration of all types of public health and medical institutions,and establish an integral grassroots medical and health service system whichcombines disease prevention and treatment. To improve service quality on the basis ofthe good coverage already attained of essential public health service. To strengthenthe three-tier, county-township-village, rural health service network and the townshiphealth centers should have the priority, and instruct patients to choose medicalinstitutes more reasonably. To strengthen health personnel construction, and improvethe assessment and incentive mechanism to fully arouse the initiative of medicalworkers. To accelerate the establishment of a negotiation mechanism and promote thereform of scientific payment system for medical and health institutions to mitigatepatients’ burden.
