Objective: The purpose of this study was to discuss Acoustic Radiation ForceImpulsein in the diagnosis of breast solid tumors.Materials and methods:1.Patientspatients referred to galactophore surgery department of liaoning Hospital wererecruited for the study. A total of196consecutive men with203lumps were enrolledfrom September2011to February2012.2were men and194were women.125lesionswere benign and78lesions were malignant.The mean age of the women examined was53±11years, ranging from21to73years. The diameter of the lesions ranged from0.32to3.31cm.2. MethodsThe ultrasound examinations were performed by standard equipment (ACUSONS2000) and broadband linear transducers (4.0~9.0MHz respectively). All the patientswere in supine position. The process began with conventional gray-scale ultrasoundimaging of the target lesions, then switched to virtual touch tissue imaging mode. Thearea and diameter of the lesion were measured on elastogram and gray-scale ultrasoundimages. The ratios of the area and diameter were calculated by the computing system ofthe ultrasound equipment.Switched to virtual touch tissue quantification(VTQ),the ratios of VTQ surveymalignant, benign breast masses and the same level breast tissue. The VTQshould take up by caculating the average of five times or three times.Results:1. Though VTI survey203breast soild tumors,the mean area ratio for benignlesions was1.02(SD0.17) and that for malignant lesions was1.32和(SD0.56). The ratios of lesion area and diameter on elastogram versus on B-mode images werestatistically different between benign and malignant lesions (P<0.05).2. The ratios of VTQ was statistically different between malignant lesions andsame level breast tissue(P<0.05)3. The ratios of VTQ was statistically different between benign and same levelbreast tissue (P<0.05)4. The ratios of VTQ was statistically different between benign and malignantlesions (P<0.05)5.The ROC (Receiver Operator Characteristic) curve was made by malignancyaccording to the VTQ.2.87was the best cutoff point of the strain ratio fordifferentiating malignant and benign breast lesions.6.The sensitivity, specificity, accuracy, positive predictive value,negativepredictive value of Spectral Doppler (SD) according to VTI were78.21%、88.80%、84.73%、81.33%、86.72%,respectively7.The sensitivity, specificity, accuracy, positive predictive value,negativepredictive value of Spectral Doppler (SD) according to VTQ were83.33%、83.25%、83.20%、75.58%、88.89%,respectively.Conclusion:1.ARFI are very useful in differentiating between malignant and benign breastmasses. Its specificity and sensitivity were both significantly high, which i mply less misdiagnosis and more confidently in the diagnosis of breast lesions.2. ARFI can quantitatively evaluate the stiffness of breast lesions, which is moreobjective than the UE scoring method. The ARFI can be used as a new supplementarymeasure for differentiation of benign and malignant breast lesions.
