| ObjectiveTo explore the relationship between oxidative stress and type2diabetes patient(T2DM) and islet β cells, insulin resistance.Meanwile, explore the influence of short-term intensive insulin pump treatment on the level oxidative stress.MethodRandomly selected60patients from our clinic or ward visits which were initialdiagnosed to T2DM on October2010to March2011,(meet the1999WHOdiagnostic criteria for diabetes) divided into experimental groups, who were treatedby insulin pump for two weeks. healthy people examined in the outpatient servicewho were given a75g oral glucosetolerance test (OGTT) were diagnosed with normal glucose tolerance (NGT) ascontrol group. fasting blood glucose (FPG),fasting serum insulin(FINS), the2hourpostprandial blood glucose (2hPG), serum glycated albumin (GA), glycerintrimyristate (TG),total cholesterol(TC), low-density lipoprotein cholesterol (LDL),high-density lipoprotein cholesterol(HDL-C), superoxide dismutase (SOD),glutathione peroxidase (GSH-Px), malonaldehyde MDA are measured andHomeostasis model assessment index for insulin resistance (HOMA-IR),Homeostasis modelassessment index for insulin sensitive (HOMA–β) are computed in all subjects.Result1.the level of FPG,2hPG,GA,TG,LDL,HOMA-IR were significant improvedin the experimental groop, while HOMA-β was significantly less than the healthycontrol group (P<0.05). 2.Antioxidative enzyme such as SOD, GSH-Px in the experimental groop was lowerthan healthy control group, while Oxidative stress index MDA was higher thanhealthy people.(P<0.01).3.Pearson correlation analysis showed the FPG,GA was positively associated withthe level of MDA,which had positive relation with HOMA-IR, while negativelyassociated with HOMA-β in the T2DM (P<0.05).4.logistic regression analysis showed SODã€MDA are all the influencing factor ofT2DM(P<0.05).5. After the CSII treatment, the level of FPG,2hPG, GA, TG and MDA weresignificant lower (P<0.01), while SOD is significantly improved (P <0.01).6. After the CSII treatment, the level of HOMA-IR is significant lower (P<0.01),while HOMA–β is significantly improved (P <0.01).7. Pearson correlation analysis showed that HOMA–β was positively associatedwith the SOD level(P<0.05).ConclusionPatients with type2diabetes have obvious glucose and lipid metabolism disorderand oxidative stress. Oxidative stress takes a significant effect on secretion of islet βcells and insulin resistance. In newly diagnosed T2DM with severe hyperglycemia,intensive insulin pump treatment could effectively control the blood sugar andfat,improve insulin resistance, restore pancreatic β cells function and increase theSOD level so that improve pancreatic β cells function caused by Oxidative stress. |
