Objective:To investigate the effect of plasma osteopontin (OPN) and carbohydrate antigen125(CA125) analysis on the related factors of value in the diagnosis of endometrial carcinoma and clinical staging, histological differentiation degree.Methods:since2011September to2012of September in Tianjin Central Obstetrics and Gynecology Hospital Department of gynecology72patients, all patients with uterine body disease, after histologic diagnosis for benign disease and malignant disease group.All patients before the operation blood samples were collected, the concentration of plasma CA125detection by electrochemical luminescence method, ELISA method was used to detect the concentration of OPN. Evaluation using a statistical method and compare the diagnostic efficacy of OPN and CA125in endometrial carcinoma, and combined detection of the two diagnostic efficiency; combined with the clinical data and histopathological diagnosis, to further explore the relationship between OPN and clinical staging, histological differentiation degree; to investigate plasma is affected by menopausal status and age factors. Data were analyzed statistically by SPSS13.0software, the non-normal distribution data, detection value as median (four bit distance)[M (Q)] said, were analyzed by non-parameter test, among a plurality of groups of data were compared with Kruskal-Wallis H test, two independent samples by Mann-Whitney U analysis of Spearman correlation non-parametric test, and clinical during the period of use; normal distribution data, detection value with x±s, t test was used for mean comparisons. P<0.05indicates significant difference.Results:there was significant difference in plasma OPN concentration in benign diseases of uterus and endometrial cancer (P<0.05), indicating that OPN can distinguish benign or malignant endometrial disease, can be used for the diagnosis of endometrial cancer; By receiver operating curve, calculate area of ROC curve of CA125index was0.532, and the area under the ROC curve was0.667OPN, at a concentration of73.72ng/ml as the cut-off value of OPN in the diagnosis of endornetrial cancer potency as the sensitivity of73.8%, specificity of60%, accuracy of68.1%, positive predictive value of72.1%, negative predictive a value of62.1%, except the specificity of CA125slightly low, other than CA125; combined detection sensitivity can be increased from73.8%to78.6%; the patients with malignant tumor according to FIGO staging discussion groups,32patients with stage Ⅰ disease, Ⅱ-Ⅲ in10cases, by statistical analysis the correlation is not significant;35cases of patients with endometrial cancer according to histopathology grade low differentiation group (G3) in6cases, in moderate differentiation group (G2) in18cases, high differentiation group (G1) in11cases, the statistical analysis, the difference was not statistically significant (P>0.05);39cases of all patients not menopause person (19cases of benign, malignant20cases),33cases of menopause (11cases of benign, malignant22cases), respectively, comparison of plasma OPN levels in postmenopausal and not menopause person in benign and malignant disease, no significant difference (P>0.05); the patients were grouped according to age^40years discussion,11Cases (4cases of benign, malignant7cases),40-60years43cases (21cases of benign, malignant22cases), more than60years in18cases (5cases of benign, malignant13cases), benign and malignant diseases among three different groups of patients before treatment OPN concentration by variance analysis, no significant difference (P>0.05).Conclusion:plasma OPN compared with CA125in the diagnosis of endometrial benign and malignant diseases, its sensitivity, accuracy and other indicators are better than the traditional CA125as a tumor marker for diagnosis of endometrial cancer, is a good tumor marker; two combined detection helps to further improve the detection sensitivity and clinical;no correlation between plasma OPN concentration and clinical staging, histological differentiation degree; efects of plasma OPN concentration was not affected by age and menopause, the diagnostic index for women of all ages to endometrial cancer.
